摘  要:目的探讨β-连接蛋白在浸润性小叶癌(ILC)与浸润性导管癌(IDC)中的鉴别诊断价值。方法60例行手术切除术的乳腺癌患者,结合浸润性不同分为IDC组(35例)、ILC组(16例)和IDC+ILC组(9例)。采用免疫组化检测三组的β-连接蛋白表达情况。分析比较β-连接蛋白在正常乳腺组织与IDC内的表达情况,β-连接蛋白于ILC内的表达情况,并分析β-连接蛋白与E-cadherin及P120表达情况的相关性。结果IDC组和IDC+ILC组的β-连接蛋白表达情况比较,差异无统计学意义(P>0.05)。ILC组与IDC+ILC组ILC成分内β-连接蛋白表达情况比较,差异无统计学意义(P>0.05)。IDC组细胞质/核异常表达率为37.1%(13/35),IDC组和ILC的β-连接蛋白表达情况比较,差异有统计学意义(P<0.05)。60例患者乳腺癌癌旁与残存的健康乳腺组织上皮内,E-cadherin与P120均呈现出连贯的细胞膜染色强阳性,阳性率均为100.0%(60/60);在IDC组35例与IDC+ILC组9例患者IDC成分内,E-cadherin与P120都表现为连续性的细胞膜染色强阳性,阳性率均为100.0%。经分析发现,E-cadherin、P120与β-连接蛋白表达程度呈正相关(r=0.994、0.872,P<0.05)。但是在ILC组16例与IDC+ILC组9例患者ILC成分内,E-cadherin的表达程度均有不断弱化趋势,甚至出现缺失,和β-连接蛋白缺失表达程度呈正相关(r=0.987、0.925,P<0.05)。但是与在IDC及正常乳腺上皮细胞上的表达形式不同,在ILD与混合型癌并发的病例内P120均呈现为细胞质棕黄色染色,β-连接蛋白表达缺失,两种标记物的表达情况比较,差异具有统计学意义(P<0.05)。结论在乳腺IDC、ILC内β-连接蛋白表达水平有别,可以作为临床鉴别诊断的依据之一。Objective To discuss the diagnostic value ofβ-connexin in differentiating infiltrating lobular carcinoma(ILC)from infiltrating ductal carcinoma(IDC).Methods A total of 60 cases of breast cancer patients who underwent surgical resection were divided into IDC group(35 cases),ILC group(16 cases)and IDC+ILC group(9 cases)according to their infiltrating characteristics.Immunohistochemistry was used to detect the expression ofβ-onnexin in the three groups.The expression ofβ-connexin in normal breast tissue and IDC,the expression ofβ-catenin in ILC was analyzed and compared,and the correlation betweenβ-catenin and E-cadherin and P120 expression was analyzed.Results There was no statistically significant difference in expression ofβ-connexin between IDC group and IDC+ILC group(P>0.05).There was no statistically significant difference in the expression ofβ-connexin in the ILC component between the ILC group and the IDC+ILC group(P>0.05).The abnormal expression rate of cytoplasm/nucleus in IDC group was 37.1%(13/35),there was statistically significant difference in expression ofβ-connexin in IDC group and ILC(P<0.05).Both E-cadherin and P120 showed strong positive cell membrane staining in the adjacent breast cancer and in the remaining healthy breast tissue of 60 patients,with a positive rate of 100.0%(60/60);in the IDC component of 35 patients in the IDC group and 9 patients in the IDC+ILC group,E-cadherin and P120 both showed continuous cell membrane staining and strong positive,with a positive rate of 100.0%.It was found that E-cadherin,P120 andβ-connexin expression were positively correlated(r=0.994,0.872,P<0.05).However,in the ILC components of 16 patients in the ILC group and 9 patients in the IDC+ILC group,the expression of E-cadherin showed a trend of weakening and even deletion,which was positively correlated with the expression ofβ-Catenin(r=0.987,0.925,P<0.05).Compared with the expression in IDC and normal mammary epithelial cells,P120 in ILD and mixed carcinoma patients showed a brownish yellow cytop
