复方苦参注射液联合顺铂治疗肝癌并发血性腹水患者的临床观察

摘　　要：目的:观察复方苦参注射液联合顺铂腹腔注射治疗肝癌并发血性腹水患者的临床疗效。方法:将87例肝癌并发血性腹水患者随机分为治疗组和对照组,治疗组患者给予复方苦参注射液联合顺铂腹腔注射治疗,对照组患者采用顺铂腹腔注射治疗,2次/周,共6次。两组患者均以中心静脉导管腹腔内置管,且尽量排尽腹水。6次腹腔注射结束后评价两组患者的近期疗效、生活质量及不良反应。结果:治疗组患者近期疗效(CR+PR/n)有效率22.7%,与对照组有效率20.9%比较,差异无统计学意义(P>0.05)。治疗组患者血性腹水颜色明显变淡,生活质量改善优于对照组(P<0.01)。治疗组患者骨髓抑制、恶心呕吐等不良反应发生率低于对照组(P<0.05)。结论:复方苦参注射液联合顺铂腹腔注射治疗肝癌并发血性腹水患者能提高生活质量,减轻化疗的不良反应,值得临床推广。Objective:To observe the clinical effect of Compound Kushen injection combined with cisplatin intraperitoneal injection in the treatment of the hepatoma bloody ascites.Methods:Eighty-seven cases of hepatoma with bloody ascites were randomly divided into treatment group and control group.In the treatment group,44 patients were given the intraperitoneal injection.The injection drug was 60ml of compound Kushen injection and 40mg of cisplatin with 40 ml of normal saline.The treatment period was two times a week with six injections.In the control group,43 patients were given intraperitoneal injection.The injection drug was 40 mg of cisplatin with 40 ml of normal saline.The treatment period was also 2 times a week with 6 injections.In both groups,a central venous catheter was inserted into the abdominal cavity,and ascites were drained as much as possible.After six times intraperitoneal injection,the short-term efficacy,quality of life,and adverse reactions of the two groups were evaluated.Results:The short term effective rate(CR+PR/n)of the treatment group was 22.7%.The short term effective rate of the control group was 20.9%.There was no significant difference between the two groups in the short term effective rate(P>0.05).After treatment,the red color of blood ascites in the treatment group was significantly lighter.The improvement of quality of life in the treatment group was better than that in the control group(P<0.01).The adverse reactions of myelosuppression,nausea,and vomiting in the treatment group were significantly less than those in the control group(P<0.05).Conclusion:There is no significant difference in the short term efficacy of the combination of Compound Kushen injection and cisplatin intraperitoneal injection in the treatment of the hepatoma bloody ascites,but it can improve the quality of life of patients and reduce the toxic and side effects of chemotherapy,which is worthy of clinical application.
