电凝钩用于胸腔镜下前纵隔肿瘤手术对手术时间和术后患者恢复速度的影响

摘　　要：目的探讨电凝钩用于胸腔镜下前纵隔肿瘤手术对手术时间和术后患者恢复速度的影响。方法选择行胸腔镜手术的86例前纵隔肿瘤患者,采用随机数字表法分为2组,各43例。2组均行胸腔镜下手术,术中对照组使用超声刀,观察组使用电凝钩。比较2组临床指标、应激反应及并发症。结果观察组手术时间[(102.75±19.98)min]短于对照组[(133.69±28.71)min]、出血量[(56.39±22.18)ml]多于对照组[(28.01±15.47)ml],差异有统计学意义(P<0.05)。2组术前、术后应激指标比较,差异无统计学意义(P>0.05);与术前相比,2组术后应激反应指标均出现升高,差异有统计学意义(P<0.05)。2组围术期均未发生严重并发症,均顺利完成手术。结论胸腔镜下前纵隔肿瘤手术治疗中使用超声刀或电凝钩均能获得较好效果,也均会引起一定的应激反应,但术后无严重并发症,安全性较高,而电凝钩手术用时更短。Objective To investigate the effect of electrocoagulation hook on the operative time and recovery rate of patients with anterior mediastinal tumor under thoracoscopic anterior mediastinal tumor.Methods 86 patients with anterior mediastinal tumor underwent thoracoscopic surgery were selected and divided into 2 groups by random number table method,43 cases in each.Thoracoscopic surgery was performed in both groups,ultrasonic knife was used in the control group,and electrocoagulation hook was used in the observation group.The clinical indicators,stress response and complications were compared between the two groups.Results The operation time of the observation group[(102.75±19.98)min]was shorter than that of the control group[(133.69±28.71)min],and the blood loss[(56.39±22.18)ml]was more than that of the control group[(28.01±15.47)ml],the difference was statistically significant(P<0.05).There was no statistically significant difference in stress index between the 2 groups before and after operation(P>0.05);No serious complications occurred in the perioperative period in the two groups,and the operation was completed successfully.Conclusion The use of ultrasonic knife or electrocoagulation hook in the surgical treatment of anterior mediastinal tumor under thoracoscopic surgery can achieve better results,and both will cause a certain stress response,but there is no serious complication after surgery,and the safety is higher,while the operation time of electrocoagulation hook is shorter.
