摘　　要：目的回顾性分析皮肤Merkel细胞癌(MCC)的临床病理形态学特征、诊断和鉴别诊断、治疗和预后。方法对1例MCC的临床表现、病理形态学特征及免疫组织化学染色情况进行分析,同时复习相关文献。结果皮肤组织病理:镜下见肿物位于真皮和皮下组织内,肿瘤细胞呈巢片状弥漫分布,细胞质少或无,细胞核小、圆形或卵圆形,染色质细腻、胡椒盐样,未见核仁,核分裂像多见。免疫组织化学染色:上皮和神经内分泌标记阳性,AE1/AE3(+),CD56(+),CK20(+),CgA(+),Syn(+)。结论MCC是一种罕见但是具有高度侵袭性的皮肤神经内分泌肿瘤,预后极差,需要与鳞状细胞癌、基底细胞癌、淋巴瘤、转移性肺小细胞癌、恶性黑色素瘤等鉴别。Objective To retrospectivly analyze clinical pathology characteristics,diagnostic and differential diagnosis,treatment and prognosis of skin merkel cell carcinoma.Methods Clinical performance,pathological morphology characteristics and immune histology expression of 1 case of merkel cell carcinoma was analyzed.At the same time,review related literature.Results Skin tissue Pathology,Under a microscope I can see tissue mainly located in leather and subcutaneous tissue,cells are spread like nest flakes and diffuse,less or no cytoplasm,small nucleus,round or egg circle,the stainise was pepper-salt-like,no nuclear kernels,nuclear division was like a common sight.Immune histology dyeing epithelial and neuroendocrine Markers was positive,AE1/AE3(+),CD56(+),CK20(+),CgA(+),Syn(+).Conclusion Merkel cell carcinoma is a rare and highly invasive skin neuroendocrine tumors,with poor prognosis.We need to identify squamous cell carcinoma,base cell carcinoma,lymphoma,metastatic lung small cell carcinoma and malignant melanoma.
