分级诊疗视角下云南某三甲医院病例转诊情况分析及思考

摘　　要：目的:分析云南某三甲医院846例转诊病例情况,为落实分级诊疗制度建设要求提供参考。方法:采集云南省T医院2016-2019年846例转诊患者信息,用定量分析法和定性分析法分析数据。结果:该院的上转患者远多于下转患者,转诊执行力不足,缺乏下转动力,转诊平台不完善,医保政策对转诊导向较弱。结论:推进医联体建设,强化下转动力,助力下级医疗机构服务能力提升,建立高效和统一的转诊平台,加强医保政策调节和导流作用,推进DRG支付方式改革,将有利于分级诊疗的深化。Objective:846 Cases study on dual referral implementation ina CertainGrade-A Tertiary Hospitalvia hospital perspective,in order toreference for both dual referral andtiered diagnosis and treatment healthy developing.Methods:To collect and obtain 846 dual referral inpatient data of Yunnan T hospital from 2016 to 2019,which were analyzed by quantitative and qualitative analysis.Results:There were upper referral inpatients more than lower,lack of dual referral execution or motivation.The referral system was immature.Medical insurance policy was lack of guiding referral.Conclusion:Be benefit to reform of tiereddiagnosis and treatment,it should be to advance medical service alliance establishment,enhanced lower referral motivation,and help primary medical institution to promote medical service,and to establish efficient and unified referralsystem platform,and to reinforce medical insurance policy adjusting and guiding,to accelerate DRG reform.
