脑小血管病患者认知功能与同型半胱氨酸及高密度脂蛋白水平的相关性研究

摘　　要：目的探讨脑小血管病患者认知功能与血清同型半胱氨酸(Hcy)及HDL水平的相关性。方法收集2018年9月~2019年12月脑小血管病患者335例,根据北京版蒙特利尔认知评估量表(MoCA)分为轻度认知功能障碍(MCI)组253例和认知功能正常组(对照组)82例,检测Hcy及HDL水平,比较各项差异。结果与对照组比较,MCI组血清Hcy水平明显升高,HDL水平明显降低(P<0.01)。血清Hcy和HDL是患者发生MCI的影响因素(P<0.01)。血清Hcy与MoCA总分、视空间及执行能力、注意力、语言、抽象思维评分呈负相关(r=-0.237、-0.136、-0.174、-0.150、-0.201,P<0.05,P<0.01);HDL与MoCA总分、视空间及执行能力、注意力、语言、抽象思维、命名及延迟回忆、定向力评分呈正相关(r=0.424、0.286、0.130、0.338、0.151、0.265、0.176、0.263,P<0.05,P<0.01)。结论Hcy和HDL与认知功能障碍相关,早期检测患者血清Hcy和HDL水平,可能有助预防和延缓认知功能障碍的发生。Objective To study the relationship of MCI and serum Hcy and HDL levels in CSVD patients.Methods Three hundred and thirty-five CSVD patients admitted to our hospital were divided into MCI group(n=253)and control group(n=82)according to their MoCA score.Their serum Hcy and HDL levels were measured.Results The serum level of Hcy was significantly higher while that of HDL was significantly lower in MCI group than in control group(P<0.01).Serum Hcy and HDL levels were the risk factors for MCI in CSVD patients(P<0.01).The serum level of Hcy was negatively related while that of HDL was positively related with the total MoCA scale score,visual space,executive competence,and attention,language,abstract thinking scores(r=-0.237,r=-0.136,r=-0.174,r=-0.150,r=-0.201;r=0.424,r=0.286,r=0.130,r=0.338,r=0.151,r=0.265,r=0.176,r=0.263,P<0.05,P<0.01).Conclusion Serum Hcy and HDL levels are related with cognitive impairment,early measurement of serum Hcy and HDL levels helps to prevent cognitive impairment in CSVD patients.
