牙周内镜下超声龈下刮治治疗慢性牙周炎患者的效果

摘　　要：目的分析牙周内镜下超声龈下刮治治疗慢性牙周炎患者的效果。方法选择2014年2月至2019年2月在我院就诊的40例慢性牙周炎患者,随机分为Ⅰ组和Ⅱ组,每组20例,Ⅰ组采取牙周内镜下超声龈下刮治,Ⅱ组采取传统手工龈下刮治,另选择20名牙周健康者设为Ⅲ组。对比Ⅰ组和Ⅱ组的治疗效果。结果治疗后,Ⅰ组和Ⅱ组患者的牙周探诊深度(PD)、牙龈出血指数(BI)以及菌斑指数(PI)较治疗前均明显下降(P<0.05),且两组的下降情况对比差异无统计学意义(P>0.05),两组治疗后的各项指标与Ⅲ组对比差异无统计学意义(P>0.05)。治疗后,两组患者的白细胞介素1(IL-1)、前列腺素E2(PGE2)对比治疗前均有显著下降(P<0.05),其中Ⅰ组IL-1与PGE2的下降程度大于Ⅱ组(P<0.05),且Ⅰ组治疗后的IL-1与PGE2与Ⅲ组对比差异无统计学意义(P>0.05)。Ⅰ组患者在治疗后的牙龈肿胀发生率与视觉模拟量表(VAS)评分明显低于Ⅱ组(P<0.05)。结论牙周内镜下超声龈下刮治治疗慢性牙周炎患者的效果显著,且安全性较高。Objective To analyze the effect of ultrasonic subgingival curettage under periodontal endoscopy in the treatment of patients with chronic periodontitis.Methods A total of 40 patients with chronic periodontitis who were treated in our hospital from February 2014 to February 2019 were selected and randomly divided into groupⅠand groupⅡwith 20 cases in each group.GroupⅠreceived periodontal endoscopic ultrasound subgingival scraping for treatment,traditional manual subgingival curettage was used in groupⅡ,and 20 healthy periodontal patients were selected as groupⅢ.Compare the treatment effect of groupⅠand groupⅡ.Results After treatment,the periodontal probing depth(PD),gingival bleeding index(BI)and plaque index(PI)of the patients in groupⅠand groupⅡwere significantly lower than before treatment(P<0.05),and the two groups decreased there was no statistically significant difference in the situation(P>0.05).There was no statistically significant difference in the indicators of the two groups after treatment with groupⅢ(P>0.05).After treatment,the levels of interleukin-1(IL-1)and prostaglandin E2(PGE2)of the two groups of patients were significantly decreased compared with those before treatment(P<0.05).Among them,the decrease of IL-1 and PGE2 in groupⅠwas greater than that in groupⅡgroup(P<0.05),and there was no significant difference in IL-1 and PGE2 between groupⅠand groupⅢafter treatment(P>0.05).The incidence of gingival swelling and Visual Analog Scale(VAS)scores of the patients in groupⅠwere significantly lower than those in groupⅡ(P<0.05).Conclusion Ultrasonic subgingival curettage under periodontal endoscopy was effective and safe in the treatment of patients with chronic periodontitis.
