摘 要:目的:探讨血浆3-硝基酪氨酸(3-NT)浓度与脑白质高信号(WMHs)患者认知功能改变的关系。方法:选取已完善头颅磁共振成像(MRI),明确WMHs患者122例。依照简易精神状态量表(MMSE)将患者分为仅WMHs组和WMHs合并认知功能改变组,分别用酶联免疫吸附试验测定两组患者血浆3-NT浓度,对结果进行分析。结果:WMHs组与WMHs合并认知功能改变组血浆3-NT浓度分别为(49.713±4.77)nmol/L、(98.4399±23.14)nmol/L,WMHs合并认知功能改变组患者血浆3-NT浓度明显高于WMHs组,差异有统计学意义(P<0.05);WMHs组与WMHs合并认知功能改变组三酰甘油(TG)分别为(1.3605±0.59147)mmol/L、(1.6229±0.78477)mmol/L,WMHs合并认知功能改变组明显高于WMHs组,差异有统计学意义(P<0.05)。将3-NT和TG纳入Logistic多元回归分析,3-NT有显著统计学意义(P<0.01),三酰甘油有统计学意义(P<0.05)。结论:TG在WMHs伴和不伴认知功能改变两组之间存在差异,但不是WMHs患者发生认知功能改变的独立危险因素;血浆3-NT浓度测定有可能作为判断WMHs患者认知功能损害的生化指标,检测3-NT的浓度有望成为预测WMHs患者认知功能减退的有效指标。Objective To discuss the relationship between plasma 3-nitrotyrosine(3-NT)concentration and cognitive function changes in patients with high white matter signal(WMHs).Method A total of 122 patients who were admitted to our hospital were included.According to mini-mental State Examination(MMSE),the patients were divided into the group with high leuk-matter signal alone and the group with high leuk-matter signal combined with cognitive decline.The plasma concentration of 3-nitrotyrosine in the two groups was measured by ELISA respectively,and the results were analyzed.Results The plasma 3-NT concentration in the WMHs group and the cognitive impairment group was(49.713±4.77)nmol/L and(98.4399±23.14)nmol/L,respectively.The plasma 3-NT concentration in the cognitive impairment group was significantly higher than that in the WMHs group(P<0.05).Triglycerides in the WMHs group and the cognitive impairment group were(1.3605±0.59147)mmol/L and(1.6229±0.78477)mmol/L,respectively,which were significantly higher in the cognitive impairment group than in the WMHs group(P<0.05).3-NT and triglycerides were included in Logistic multivariate regression analysis,and 3-NT and triglycerides were statistically significant(P<0.01),while triglycerides were statistically significant(P<0.05).Conclusion There were differences in triglycerides between groups with and without cognitive function changes,but they were not independent risk factors for cognitive function changes in patients with high white matter signal.Determination of plasma 3-NT concentration may be a biochemical indicator of cognitive impairment in patients with high white matter signal,and detection of 3-NT concentration is expected to be an effective indicator of cognitive impairment in patients with high white matter signal.
