电视胸腔镜下肺叶切除术治疗高龄非小细胞肺癌患者的临床研究

摘　　要：目的研究电视胸腔镜下肺叶切除术治疗高龄非小细胞肺癌(NSCLC)患者的临床效果。方法选取2019年3月至2020年3月期间我院收治的168例高龄NSCLC患者,根据治疗方法不同分为对照组(n=83)和观察组(n=85)。对照组采用传统开胸肺叶切除术治疗,观察组采用电视胸腔镜下肺叶切除术治疗,比较两组的手术相关指标及并发症发生情况。结果两组的淋巴结清扫量比较无统计学差异(P>0.05)。观察组的手术时间、住院时间短于对照组,切口长度小于对照组,术中出血量少于对照组(P<0.05)。观察组的并发症发生率为5.88%,明显低于对照组的21.69%(P<0.05)。结论对高龄NSCLC患者来说,电视胸腔镜下肺叶切除术与传统开胸肺叶切除术的淋巴结清扫量相当,但电视胸腔镜下肺叶切除术可明显缩短手术时间及住院时间,减少术中出血量,缩小手术切口,且并发症发生率较低,值得临床推广应用。Objective To study the clinical effect of video-assisted thoracoscopic lobectomy in the treatment of elderly patients with non-small cell lung cancer(NSCLC).Methods 168 elderly NSCLC patients admitted to our hospital from March 2019 to March 2020 were selected and divided into control group(n=83)and observation group(n=85)according to different treatment methods.The control group was treated with traditional thoracotomy lobectomy,and the observation group was treated with video-assisted thoracoscopic lobectomy.The operation-related indicators and complications of the two groups were compared.Results No statistical difference was found in the amount of lymph node dissection between the two groups(P>0.05).The operation time and hospitalization time of the observation group were shorter than those of the control group,the incision length was smaller than that of the control group,and the intraoperative blood loss was less than that of the control group(P<0.05).The incidence of complications in the observation group was 5.88%,significantly lower than 21.69%in the control group(P<0.05).Conclusions For elderly NSCLC patients,video-assisted thoracoscopic lobectomy has the similar amount of lymph node dissection with traditional thoracotomy lobectomy,but video-assisted thoracoscopic lobectomy can significantly shorten the operation time and hospitalization time,reduce the intraoperative blood loss,and reduce the surgical incision,with lower incidence of complications,which is worthy of clinical promotion and application.
