血清腺苷脱氨酶检测对急性淋巴细胞白血病患者的诊断价值分析

摘　　要：目的分析血清腺苷脱氨酶(ADA)检测对急性淋巴细胞白血病患者的诊断价值。方法选择2018年6月至2020年6月我院收治的疑似急性淋巴细胞白血病患者100例,均检测血清ADA水平,并进行临床综合诊断。以临床综合诊断作为金标准,分析血清ADA诊断急性淋巴细胞白血病的临床价值,并分析确诊者肝功能对血清ADA水平的影响。结果 100例疑似患者中检出32例急性淋巴细胞白血病患者,占比为32.00%。ROC曲线结果显示,血清ADA用于诊断急性淋巴细胞白血病的AUC、灵敏度、特异度、准确度分别为0.915 (诊断性能较高)、 87.50%(28/32)、 95.59%(65/68)、 93.00%(93/100)。32例急性淋巴细胞白血病患者中肝功能正常组ADA水平为(19.21±3.17) U/L,低于肝功能异常组的(29.89±4.64) U/L,差异有统计学意义(P <0.05)。结论血清ADA检测在急性淋巴细胞白血病患者诊断中具有较高的灵敏度、特异度、准确度,且在不同肝功能状态下表达有所差异,利于临床判断患者肝功能情况。Objective To analyze the diagnostic value of serum adenosine deaminase(ADA) in patients with acute lymphoblastic leukemia. Methods 100 patients with suspected acute lymphoblastic leukemia admitted to our hospital from June 2018 to June 2020 were selected. Serum ADA levels were detected and comprehensive clinical diagnosis was performed. Taking comprehensive clinical diagnosis as the gold standard, the clinical value of serum ADA in the diagnosis of acute lymphoblastic leukemia was analyzed, and the influence of liver function on serum ADA level was analyzed. Results Among 100 suspected patients, 32 cases were detected with acute lymphoblastic leukemia, accounting for 32.00%. ROC curve results showed that the AUC, sensitivity, specificity and accuracy of serum ADA in the diagnosis of acute lymphoblastic leukemia were 0.915(with higher diagnostic efficiency), 87.50%(28/32), 95.59%(65/68) and 93.00%(93/100), respectively. Among the 32 patients with acute lymphoblastic leukemia, the ADA level of the normal liver function group was(19.21 ± 3.17) U/L, lower than(29.89 ± 4.64) U/L of the abnormal liver function group(P <0.05). Conclusions Serum ADA detection has higher sensitivity, specificity and accuracy in the diagnosis of acute lymphoblastic leukemia patients, and the expression is different under different liver function states, which is helpful for clinical judgment of liver function.
