右眼脉络膜黑色素瘤1例

摘　　要：47岁女性,急诊因"右眼球突出伴视力丧失、眼红、眼痛10 d"入院。体格检查发现右眼无光感,右眼球突出,各方向运动均受限,上睑红肿,结膜明显充血水肿,角膜点尘状KP(+),前房浅,Tyndall(+),晶状体前移呈黄白色混浊,眼压升高。影像学提示患者右眼球内占位性病变,因此入院后给予手术摘除眼球。术后病理提示右眼脉络膜黑色素瘤。讨论体会:以眼眶蜂窝织炎为表现的眼球突出以及继发性眼球疼痛是脉络膜黑色素瘤的一种临床表现,应避免仅施行抗炎治疗而错失切除肿瘤的手术时机。A 47-year-old female presented to emergency with a 10-day history of proptosis,sudden vision loss,redness and pain in the right eye.Ocular examination revealed no light perception in the right eye,prominent right eyeball,restricted movement in all directions,swelling of the upper eyelid,obvious conjunctival hyperemia and edema,dusty KP(+),shallow anterior chamber,Tyndall(+),lens anterior shift and yellowishwhite turbid,and intraocular pressure is elevated.Imageological examinations demonstrated an intraocular space-occupying lesion in the right eye.Thus,an ophthalmectomy of right eye was performed and a diagnosis of choroid melanoma was confirmed by histopathological examination.Discussion and experience:Orbital cellulitis presenting as proptosis and secondary eye pain is one of the clinical manifestations of choroidal melanoma.Thus,the insolated anti-inflammation treatment should be avoided so as to miss the opportunity of tumor resection.
