摘　　要：根据ISBAR标准化沟通模式理论,构建医师信息化ISBAR交接班系统,替代纸质书写的医师交接班记录,在系统中形成完整的电子交接班数据库。信息化ISBAR交接班系统的实施,提高了医师交接班记录书写效率和内涵质量,为诊疗工作的连续性和完整性提供保障,为职能部门监管和质量改进提供依据。According to theory of Identify-Situation-Background-Assessment-Recommendation(ISBAR)standardized communication mode,the ISBAR information handover system for physicians was constructed.The paper-written handover records of the physicians were replaced by complete electronic handover database in the system.The implementation of the ISBAR information handover system improved the efficiency and quality of handover records of the physicians,ensuring the continuity and integrity of diagnosis and treatment,and providing reference for the supervision of functional departments and for quality improvement.
