显微镜下隐裂牙根管治疗的诊断与疗效

摘　　要：目的:探讨显微镜下隐裂牙根管治疗的诊断与疗效。方法:选取2017年6月~2018年6月于本院因牙隐裂导致牙髓炎的患者60例,按病情严重程度分为A组(隐裂纹深度未达髓腔)20例(20颗患牙)、B组(隐裂纹深度达髓腔,未及髓室底)20例(20颗患牙)、C组(隐裂纹深度达髓室底)20例(20颗患牙)。三组患者均行显微镜下隐裂牙根管治疗,根管治疗后无症状的行全冠修复,一年复查时评价临床疗效。结果:A组成功17例(85.0%),有效2例(10.0%),失败1例(5.0%)。B组成功12例(60.0%),有效5例(25.0%),失败3例(15.0%)。C组成功2例(10.0%),有效3例(25.0%),失败13例(65.0%)。A、B组根管治疗冠修复后成功率显著高于C组,差异有统计学意义(P<0.05)。结论:牙隐裂越早行根管治疗,效果越好,当患牙隐裂纹达髓室底时,治疗的成功率低。Objective:To investigate the diagnosis and effect of root canal therapy for cryptofissure under microscope.Methods:60 patients with tooth cracked cause pulpitis from June 2017 to June 2018 in our hospital,according to the illness severity were divided into group A(hidden crack depth of medullary cavity)20 cases(20 with teeth),group B(hidden crack depth of pulp cavity,not the bottom of the pulp chamber)20 cases(20 with teeth),and group C(hidden crack depth reaches the bottom of the pulp chamber)20 cases(20 with teeth).All the patients in the three groups were treated with root canal therapy under microscope.After root canal therapy,the whole crown was repaired without symptoms,and the clinical efficacy was evaluated at one-year reexamination.Results:In group A,17 cases(85.0%)were successful,2 cases(10.0%)were effective,and 1 case(5.0%)were failed.In group B,12 cases(60.0%)were successful,5 cases(25.0%)were effective,and 3 cases(15.0%)were failed.In group C,2 cases(10.0%)were successful,3 cases(25.0%)were effective,and 13 cases(65.0%)were failed.The success rate of root canal treatment crown repair in group A and group B were significantly higher than that in group C,with statistically significant difference(P<0.05).Conclusion:The earlier the root canal treatment for fissure,the better the effect.When the alveolar fissure reaches the bottom of pulp chamber,the success rate of treatment is low.
