摘  要:目的探讨基于血氧水平依赖(BOLD)功能磁共振成像(fMRI)的被动手运动刺激对中央区胶质瘤运动功能区术前定位的有效性.方法选取病灶位于中央区的胶质瘤患者15例,分别对患者进行主动手运动及被动手运动的任务刺激,采用独立成分分析方法观察运动功能区的激活范围及强度的变化.结果被动手运动与主动手运动fMRI激活范围及强度相似,通过统计学比较显示2种模式下左侧主要感觉运动皮层区(LPMC)、右侧主要感觉运动皮层区(RPMC)及辅助运动皮层区(SMA)的激活强度未见明显统计差异(P>0.05).结论被动运动可以作为一种有效的补充手段来定位运动功能区,帮助神经外科医师最大程度切除脑肿瘤,保护运动功能区.Objective TO investigate the effectiveness of preoperative localization of the motor functional areas in patients with glioma located in the central region using blood oxygen level dependent(BOLD)functional magnetic resonance imaging(fMRI)during passive hand movement stimulation.Methods Fifteen patients with gliomas located in the central region were selected.Al patients underwent the active hand movement and passive hand movement tasks/The independent component analysis method was used to observe changes of activation range and intensity in the motor functional arca.Results The range and intensity of fMRI activation showed similar patterns during passive and active hand movement tasks.Between the two task modes,no statistically significant difference of activation intensity was showed(P>0.05)in the left primary motor cortex(LPMC),right primary motor cortex(RPMC)and supplement motor cortex(SMA).Conclusion Passive hand movement can be used as an effective supplementary method to locate motor functional areas,which can assist neurosurgeons to achieve maximal removal of brain tumors while preserving motor functional areas.
