摘　　要：目的研究动脉自旋标记(ASL)MR灌注成像对脑膜瘤病理分型鉴别诊断及临床应用价值.方法对经手术病理证实的45例脑膜瘤患者术前行三维动脉自旋标记(3DGASL)、MRI平扫及增强序列扫描.测量ASL MR血流灌注图中肿瘤实质部分最大肿瘤血流量(TBF)和对侧镜像脑血流量(CBF),采用SPSS25.0统计软件进行分析,用两样本独立t检验比较各个参数值,P<0.05为差异有统计学意义.结果上皮型肿瘤TBF平均值高于镜像区,过渡型肿瘤TBF平均值与镜像区无明显差异,纤维型肿瘤TBF平均值低于镜像区.单因素方差分析示:脑膜瘤3种亚型瘤体实质最大TBF值平均数间差异有统计学意义(P<0.05).结论不同分型脑膜瘤ASL MR血流灌注图表现具有差异性,灌注参数具有特异性,ASL MR灌注成像可预估脑膜瘤分型,对临床手术及治疗方式提供参考意义.Objective To investigate the value of arterial spin labeling(ASL)MR perfusion imaging in the differential diagnosis of pathological type of meningiomas and its advantages in clinical application.Methods Preoperative thrce-dimicnsional ASL(31-ASL),MRI plain and enhanced imaging were performed on 45 patients with meningiomas confirmed by surgery and pathology.The maximum tumor blood flow(TBF)in the solid portion of the tumor and the contralateral mirror cerebral blood flow(CBF)in the ASL MR blood perfusion imaging were measured and analyzed using SPSS 25.0 statistical software/Two independent sample i-tests were used to compare the values of each parameter with P<0.05 considered as statistically significant.Results The mean TBF of epithelial tumors was higher than that of mirror regions;the mean T'BF of transitional tumors was not significantly different from that of mirror regions;and the mean TBF of fibrous tumors was lower than that of mirror regions.One-way ANOVA showed that there was a statistically significant difference in the maximum TBF values of the solid portion of the three subtypes of meningiomas(P<0.05).Conclusion Different patterns are observed in the ASL MR blood perfusion imaging of different types of meningioma,and the perfusion parameters are specific.ASL MR perfusion imaging shows the ability to predict the classification of meningiomas and provide a reference for clinical surgery and treatment.
