摘  要:目的分析转化生长因子-β1(TGF-β1)、Smad-7及血管内皮生长因子(VEGF)在脑胶质瘤中表达及与病理特征、预后的关系。方法收集57例2017年5月至2019年1月于本院神经外科行肿瘤切除术的胶质瘤患者,据预后情况分为生存组(n=41)及死亡组(n=16)。同时选取50例无任何肿瘤性疾病的脑外伤患者的新鲜脑组织,作为对照组。分析TGF-β1、Smad7及VEGF与脑胶质瘤患者各病理参数的相关性,采用多元Logistic回归分析影响脑胶质瘤患者预后生存的危险因素,绘制KaplanMeier生存曲线研究TGF-β1、Smad7及VEGF对患者预后生存的影响。结果实验组TGF-β1、Smad7、VEGF高表达率较对照组高(P<0.05)。TGF-β1、Smad7及VEGF表达与脑胶质瘤患者组织分化程度、WHO分级有显著相关性(P<0.05)。死亡组TGF-β1、Smad7、VEGF高表达率较生存组高(P<0.05)。组织中低分化、WHO分级:Ⅲ+Ⅳ级、TGF-β1、VEGF及Smad7高表达是影响脑胶质瘤患者预后生存的独立危险因素(P<0.05)。TGF-β1低表达组、Smad7低表达组、VEGF低表达组平均生存时间较高表达组长(P<0.05)。结论TGF-β1、Smad7、VEGF在脑胶质瘤患者中呈高表达状态,可VEGF在脑胶质瘤中低表达TGF-β1、Smad7蛋白高表达及VEGF蛋白低表达参与脑胶质瘤临床生物学进展,并与患者预后生存关系密切。Objective To analyze the expression of transforming growth factor-β1(TGF-β1),Smad-7 and vascular endothelial growth factor(VEGF)in glioma and their relationship with pathological characteristics and prognosis.Methods The tumor tissues of 60 patients with glioma who underwent tumor resection in the Department of Neurosurgery of the Affiliated Hospital of Chengde Medical College from May 2017 to November 2019 were collected.In the end,57 patients meeting the inclusion and exclusion criteria of this study were regarded as the experimental group.According to the prognosis during the follow-up period,they were divided into survival group(n=41)and death group(n=16).At the same time,50 fresh brain tissues of brain trauma patients without any tumor diseases were selected as the control group.The expressions of TGF-β1,Smad7 and VEGF in the experimental group and the control group were compared.The correlation between TGF-β1,Smad7 and VEGF and the pathological parameters of patients with glioma was analyzed,and the survival and death of all patients with glioma during the follow-up period were recorded.Multivariate Logistic regression was used to analyze the risk factors that affect the prognosis of patients with glioma,and the Kaplan-Meier survival curve was drawn to study the effects of TGF-β1,Smad7 and VEGF on the prognosis of patients.Results The high expression rate of TGF-β1,Smad7 and VEGF in the experimental group was higher than that in the control group(P<0.05).The expression of TGF-β1,Smad7 and VEGF was significantly correlated with the degree of tissue differentiation and WHO classification in patients with glioma(P>0.05).The high expression rate of TGF-β1,VEGF and Smad7 in the death group were higher than those in the survival group(P<0.05).Poorly differentiated tissue,TNM staging:Ⅲ+Ⅳ,high expression of TGF-β1,VEGF,and Smad7 were independent risk factors affecting the prognosis survival of patients with glioma(P<0.05).The average survival time of TGF-β1 low expression group,Smad7 low expression gr
