摘 要:目的:研究不同根充方式和清洗方法对纤维桩粘接强度的影响。方法:收集因正畸治疗需要或者因牙周炎拔除的离体下颌单根管前磨牙48颗,常规根管预备后随机分为两组,每组24颗。A组仅根充根尖1/3,B组热牙胶垂直加压充填整段根管。桩道预备后每组再根据根管清洗方法分为3个亚组(每组8颗),分别采用3种冲洗剂及冲洗剂组合对桩道进行冲洗。A1、B1组:0.9%NaCl+17%EDTA溶液;A2、B2组:0.9%NaCl+17%EDTA+3%NaClO溶液冲洗;A3、B3组:0.9%NaCl+17%EDTA+5.25%NaClO溶液冲洗。采用Rely XTMU200树脂粘接剂黏固纤维桩,1周后行薄片推出实验。结果:A1组纤维桩粘接强度为(19.14±3.99)MPa,A2组为(15.22±3.08)MPa,A3组为(14.15±3.26)MPa,B1组为(16.43±3.16)MPa,B2组为(14.17±2.99)MPa,B3组为(13.20±3.16)MPa。两两组间比较采用LSD法分析表明,A1组的粘接强度明显高于A2和A3组;B1组的粘接强度明显高于B2和B3组(均P<0.05);A1组与A2组、A3组比较,差异有统计学意义(P<0.05),A2组与A3组比较,差异无统计学意义(P>0.05);B1组与B2组、B3组比较,差异有统计学意义(P<0.05),B2组与B3组比较,差异无统计学意义(P>0.05)。结论:用0.9%NaCl+17%EDTA溶液清洗桩道,根充根尖1/3会提高纤维桩的剪切粘接强度。根充方式相同的情况下,17%EDTA溶液桩道冲洗效果最佳,在临床上粘接纤维桩可做参考。Objective:To study the effect of different root filling methods and cleaning methods on the bonding strength of fiber posts.Methods:Fourty-eight isolated mandibular single canal premolars extracted were collected due to orthodontic treatment or periodontitis.After conventional root canal preparation,they were randomly divided into two groups,24 teeth in each group.Group A only had 1/3 of the root tip,group B hot gutta-percha fills the entire root canal with vertical pressure.After the piles were prepared,each group was divided into three subgroups(8 teeth in each group).According to the root canal cleaning method,and the piles were washed with three kinds of rinsing agents and combinations of rinsing agents.A1,B1 group was given 0.9%NaCl+17%EDTA solution.A2,B2 group was given 0.9%NaCl+17%EDTA+3%NaClO solution alternate washing.A3,B3 group was given 0.9%NaCl+17%EDTA+5.25%NaClO.RelyXTMU200 resin adhesive was used to cement the fiber post,1 week later,the thin-sheet push-out experiment was conducted.Results:The bond strengths of A1 was(19.14±3.99)MPa,A2(15.22±3.08)MPa,A3(14.15±3.26)MPa.B1(16.43±3.16)MPa,B2(14.17±2.99)MPa,B3(13.20±3.16)MPa.The comparison between the two groups using the LSD method showed that the bonding strength of the A1 group was significantly higher than that of the A2 and A3 groups,the bonding strength of the B1 group was significantly higher than that of the B2 and B3 groups(P<0.05).The bonding strength between A1 group,A2 group and A3 group is statistically significant(P<0.05),no significant difference was observed between A2 group and the A3 group(P>0.05).No significant difference was observed between B2 group and B3 group(P>0.05).Conclusion:When the 0.9%NaCl+17%EDTA solution cleans the post,the root filling of the root tip 1/3 will increase the shear bonding strength of the fiber post.With the same root filling method,17%EDTA solution is an effective post flushing agent,and it can be used as a reference for bonding fiber posts in clinical practice.
