康复新液联合盐酸米诺环素对固定正畸牙周炎患者龈沟液MMP-8和MMP-13水平的影响

摘　　要：目的:探讨固定正畸牙周炎患者经康复新液联合盐酸米诺环素治疗的效果,为未来该类疾病治疗方案的制定提供参考。方法:选取2018年6月~2019年12月于本院接受固定正畸治疗后发生牙周炎的患者144例,采用随机数字表法分为观察组和对照组,每组72例。全部患者均接受口腔卫生健康教育联合盐酸米诺环素(注满牙周袋,1次/周)治疗,观察组患者在此基础上联合使用康复新液(含漱,10 ml/次,5 min/次,tid)治疗,均治疗4周。分别于治疗前、治疗4周后,检测两组龈沟液基质金属蛋白酶-8(MMP-8)、基质金属蛋白酶-13(MMP-13)水平;检测龈沟液中炎性因子[白细胞介素-8(IL-8)、白细胞介素-10(IL-10)、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子(TNF-α)]水平;分别观察并记录两组治疗期间不良反应发生情况。结果:治疗4周后,两组龈沟液MMP-8和MMP-13水平均较治疗前降低,且观察组低于对照组,具有统计学差异(P <0.05);治疗4周后,两组龈沟液中IL-8、hs-CRP和TNF-α水平均较治疗前降低,且观察组低于对照组;IL-10水平较治疗前升高,且观察组高于对照组,具有统计学差异(P <0.05);两组菌群失调、消化道反应、肝损害、肾损害和过敏反应发生率进行对比,无统计学差异(P> 0.05)。结论:固定正畸牙周炎患者经康复新液联合盐酸米诺环素治疗获益明显,患者龈沟液MMP-8和MMP-13水平均改善,有利于减轻炎症反应,且联合用药不会增加不良反应发生率、安全性较高,有较高的临床应用价值。Objective:To investigate the effects of Kangfuxin solution combined with minocycline hydrochloride in treatment of periodontitis patients with fixed orthodontics,and to provide a reference for the future treatment of such diseases.Methods:144 patients with periodontitis after fixed orthodontic treatment in the hospital from June 2018 to December 2019 were selected and divided into observation group and control group according to random number table method,with 72 cases in each group.All patients received oral hygienic health education combined with minocycline hydrochloride(filled the periodontal pocket,1 time/week).And on this basis,observation group was treated with additional Kangfuxin solution(gargle,10 ml/time,5 min/time,3 times/d),with 4 weeks of treatment.Before tand after 4 weeks of treatment,the levels of gingival crevicular fluid matrix metalloproteinase-8(MMP-8)and matrix metalloproteinase-13(MMP-13)in two groups were detected;The levels of inflammatory factors[interleukin-8(IL-8),interleukin-10(IL-10),high-sensitivity C-reactive protein(hs-CRP),tumor necrosis factor(TNF-α)]in gingival crevicular fluid were detected.The incidence of adverse reactions in two groups was observed and recorded.Results:The levels of MMP-8 and MMP-13 in gingival crevicular fluid in two groups after 4 weeks of treatment were lower than before treatment,with lower results found in the observation group than in the control group(P<0.05);The levels of IL-8,hs-CRP and TNF-αin gingival crevicular fluid in two groups after 4 weeks of treatment were lower than before treatment,with lower results found in the observation group than in the control group.The level of IL-10 was higher than before treatment,with higher result found in the observation group than in the control group(P<0.05).There was no statistical difference in the incidence of flora imbalance,gastrointestinal reaction,liver damage,kidney damage and allergic reaction between the two groups(P>0.05).Conclusion:Kangfuxin solution combined with minocycline hydrochloride in
