腹腔镜与开腹手术下结肠癌肠梗阻患者血清炎症因子及感染状况分析

摘　　要：目的:探讨腹腔镜与开腹手术对结肠癌肠梗阻患者血清炎症因子与感染状况的影响。方法:选取某院接受治疗的118例结肠癌肠梗阻患者,根据手术方式将患者分为2组,64例接受腹腔镜手术的患者为观察组,54例接受开腹手术的患者为对照组。比较2组患者手术相关指标、血清炎症因子水平、感染发生率、并发症发生率、术后恢复指标及病原菌分布情况。结果:观察组患者切口长度、术中出血量、术后排气时间均短于对照组,差异有统计学意义(P<0.05);观察组患者术后24、72 h各项血清炎症因子指标均低于对照组,差异有统计学意义(P<0.05);观察组患者感染发生率、并发症发生率均低于对照组,差异有统计学意义(P<0.05);观察组患者术后指标的恢复优于对照组(P<0.05);感染患者病原菌以革兰阴性菌和革兰阳性菌为主。结论:腹腔镜手术治疗结肠癌肠梗阻有利于患者术后恢复,能够降低患者血清炎症因子水平及术后感染的发生率。Objective:To investigate the effects of laparoscopic surgery and open surgery on serum inflammatory factors and infection status in patients with colonic intestinal obstruction.Methods:A total of 118 patients with colon cancer obstruction were enrolled.The patients were divided into two groups according to the patients'surgical procedure.64 patients undergoing laparoscopic surgery were in the observation group,and 54 undergoing open surgery were in the control group.Relative surgical indexes,levels of serum inflammatory factors,incidences of infection and complications,postoperative recovery indexes,and distribution of pathogens in the infected patients were compared between the two groups.Results:The incision length,intraoperative blood loss,and postoperative exhaust time were less or shorter in the observation group than in the control group(P<0.05).24h and 72h after the operation,the indexes of different serum inflammatory factors in the observation group were lower than those in the control group(P<0.05).The incidence of infection and the postoperative complication rate in the observation group were lower than those in the control group(P<0.05).The postoperative recovery index was better than that of the control group(P<0.05).Gram-negative bacteria and Gram-positive bacteria were the main pathogens.Conclusion:Laparoscopic surgery for colon cancer patients with intestinal obstruction is beneficial to postoperative recovery,which can reduce serum inflammatory factor levels and decrease the incidence of infection.
