摘 要:目的:深入探讨超声对腮腺肿块良恶性的鉴别诊断及临床应用价值。方法:选取我院从2015年2月—2020年1月诊治的75例腮腺肿块患者作为研究对象。这些患者均经过超声检查,临床上均进行手术切除治疗,以术后的病理检测结果作为"金标准",统计分析超声诊断腮腺肿块良恶性的敏感性、特异性和准确度。另外,比较超声和病理对各种类型良恶性腮腺肿块的检出率。结果:以术后的病理检查结果作为"金标准",超声诊断腮腺肿块良恶性的特异性、敏感性和准确度分别约为98.55%、66.67%和96.00%。分析对比结果显示:超声有1例腺泡细胞癌误诊为囊肿,误诊率50.00%;1例黏液表皮样癌误诊为多形性腺瘤,误诊率33.33%,1例腮腺内的淋巴结反应性增生被误判为恶性;良性肿块中,5例基底细胞腺瘤和3例腺淋巴瘤误诊为多形性腺瘤,误诊率分别为50.00%、25.00%,还有1例面神经纤维瘤误诊为多形性腺瘤。结论:超声对腮腺肿块的检出率100%,对腮腺良恶性肿块也具有较高的检出率,特异性和准确性较高,在腮腺肿块良恶性鉴别诊断中具有较高的临床应用价值,对临床研究及诊治有很高的实用价值。Objective:To evaluate the value of ultrasonography in the differential diagnosis of benign and malignant parotid masses.Methods:A total of 75 patients with parotid masses were selected from February 2015 to January 2020.The sensitivity,specificity and accuracy of ultrasonography in the diagnosis of benign and malignant parotid masses were analyzed according to the"gold standard"of postoperative pathology.In addition,the detection rates of various types of benign and malignant parotid masses by ultrasound and pathology were compared.Results:The specificity,sensitivity and accuracy of ultrasonography in the diagnosis of benign and malignant parotid masses were 98.55%,66.67%and 96.00%,respectively.The results showed that one case was misdiagnosed as cyst.One case as mucoepidermoid carcinoma was misdiagnosed as pleomorphic adenoma.One case of reactive hyperplasia of lymph nodes in parotid gland was misdiagnosed as malignant.Among the benign tumors,five basal cell adenomas and three adenolymphomas were misdiagnosed as pleomorphic adenomas,the misdiagnosed rates were 50.00%and 25.00%respectively,and 1 neurofibromatosis was misdiagnosed as pleomorphic adenoma.Conclusion:The detection rate of parotid gland mass by ultrasonography was 100%,and the detection rate of benign and malignant parotid gland mass was also high.The specificity and accuracy were high.It has high practical value for clinical research,diagnosis and treatment.
