摘　　要：目的探讨前路开眶治疗眼眶肿瘤的临床疗效。方法收集接受眶内肿瘤治疗的97例眼眶肿瘤患者的临床资料,分析前路开眶肿瘤切除的治疗效果、术后并发症及患者预后情况。结果95例眼眶肿瘤患者均完整摘除肿瘤;2例眼眶肿瘤患者的肿瘤与周围组织粘连紧密,为保存视力而行大部分切除术。术中肿瘤位置及粘连情况与影像学检查结果基本一致。术后视力增进者28例,无变化者53例,视力下降者16例。术后随访3~15个月,无肿瘤复发病例。所有患者均未发生视力丧失、眼球固定、感染等严重并发症。97例眼眶肿瘤患者中,79例患者术后出现不同程度的眼睑肿胀淤血,术后7~5天消退;17例患者术后出现眼球运动障碍、暂时性复视,经指导眼球运动后,1个月内眼球运动恢复正常,复视消失。6个月后仍有1例眼球极限运动受限和1例瞳孔向心性扩大。结论前路开眶治疗能有效改善眼眶肿瘤患者的视力与眼球突出度,且术后并发症少,是治疗眼眶肿瘤的有效方式,但应在临床及影像学检查的指导下选择合适的眼眶手术入路适应证。Objective To explore the clinical efficacy of anterior open-orbital treatment in orbital tumors.Method The clinical data of 97 patients undergoing intraorbital tumor treatment were collected to analyze the efficacy,postoperative complications and prognosis of anterior open-orbital tumor resection.Result 95 patients of orbital tumors received complete resection of tumors,and 2 patients received resection of most tumors to preserve vision due to the tight adhesion of tumors to surrounding tissues.Intraoperative tumor location and adhesion were basically consistent with imaging examination.Postoperative visual acuity improved in 28 cases,remained unchanged in 53 cases,and decreased in 16 cases with no tumor recurrence cases after 3-15 months postoperative follow up.There were no serious complications of vision loss,eye fixation and infection.79 patients had different degrees of postoperative eyelid swelling and congestion which resolved 5-7 days after operation,and 17 patients had postoperative eye movement dysfunction and temporary diplopia,which recovered for eye movement and diplopia within 1 month after eye movement guidance.After 6 months,there was still 1 case of limited eye movement and 1 case of dilated pupil.Conclusion Anterior open-orbital treatment can effectively improve the vision and eyeball protrusion of patients with orbital tumors with fewer postoperative complications,which is an effective way to treat orbital tumors,but it is necessary to select the appropriate indication according to clinical and imaging examinations.
