复方血栓通胶囊对氯吡格雷抵抗急性脑梗死的疗效及神经功能的影响

摘　　要：目的探讨复方血栓通胶囊对氯吡格雷抵抗急性脑梗死的疗效及神经功能的影响,为临床诊疗提供参考依据。方法选取2015年7月—2018年10月住院治疗的急性脑梗死患者352例进行研究。挑选出存在氯吡格雷抵抗患者100例,按照1∶1比例随机分两组,各50例。对照组采用氯吡格雷治疗,观察组在此基础上联合复方血栓通胶囊治疗。比较治疗后效果,记录治疗前、后患者美国国立卫生研究院卒中量表(NIHSS)评分、生活质量变化,统计不良反应。结果治疗后,观察组总有效率明显高于对照组,差异有统计学意义(P <0.05)。与治疗前比较,两组患者的NIHSS评分均明显降低,且观察组[(3.56±0.61)分]明显低于对照组[(4.68±0.58)分],差异有统计学意义(P <0.05)。治疗后,两组患者身体机能、社交能力、身体角色、情感角色、心理健康、活力、身体疼痛、总体健康等评分均明显改善,且观察组明显低于对照组,差异有统计学意义(P <0.05)。两组不良反应相比,差异无统计学意义(P> 0.05)。结论复方血栓通胶囊治疗氯吡格雷抵抗疗效显著,可有效改善伴有氯吡格雷急性脑梗死患者的神经功能,提高患者生活质量,值得临床推广使用。Objective To explore the effect of Compound Xueshuantong Capsule on the resistance of clopidogrel of acute cerebral infarction and its neurologic function,so as to provide reference for clinical diagnosis and treatment.Methods A total of 352 patients with acute cerebral infarction hospitalized from July 2015 to October 2018 were selected for study.Totally 100 patients with clopidogrel resistance were selected and randomly divided into two groups according to the ratio of 1 ∶ 1,50 cases in each group.The control group was treated with clopidogrel,and the observation group was treated with Compound Xueshuantong Capsule.The effects after treatment were compared,and the National Institutes of Health Stroke Scale( NIHSS) score,quality of life changes and adverse reactions were recorded before and after treatment.Results After treatment,the total effective rate in the observation group was higher than that in the control group,and the difference was statistically significant( P < 0.05).Compared with those before treatment,the NIHSS scores of the two groups were significantly reduced,and the observation group’ s score[( 3.56 ± 0.61) points]was significantly lower than that of the control group[( 4.68 ± 0.58) points].The difference was statistically significant( P < 0.05).After treatment,the scores of physical function,social ability,physical role,emotional role,mental health,vitality,physical pain and overall health of the two groups of patients were reduced,and those of the observation group were significantly lower than those of the control group.The difference was statistically significant( P < 0.05).There were no significant differences in adverse reactions between the two groups( P > 0.05).Conclusion Compound Xueshuantong Capsule has significant curative effect on clopidogrel resistance,which can reduce the platelet aggregation rate in patients with acute cerebral infarction with clopidogrel resistance and improve neurological function and the quality of life of patients.
