S3镍钛锉用于干髓术后患牙根管治疗期间急症的观察

摘　　要：目的比较两种不同器械对干髓术后牙髓炎及根尖周炎患牙根管治疗期间疼痛的影响。方法选择江南大学附属医院口腔科2014年1月—2016年12月干髓术后需要根管治疗的患者150例,分为两组,各75例,对照组患者采用手动不锈钢K锉,逐步后退法进行根管预备,观察组患者采用S3机用镍钛锉,冠向下根管预备方法进行根管预备。分别对两组患者进行根管预备术后随访,记录并比较两组患者根管预备后疼痛发生情况。结果根管预备后,观察组有24颗患牙发生EIAE,发生率为32%;对照组有48颗患牙发生EIAE,发生率为64%,观察组根管预备术后疼痛Ⅰ级(17.3%)Ⅱ级(10.6%)Ⅲ级(4%)发生情况均优于对照组(Ⅰ级29.3%,Ⅱ级24%,Ⅲ级10.7%),统计学分析χ2=15.818,P<0.05,差异有统计学意义。结论和传统手动K锉根管预备相比,S3镍钛锉在干髓术后牙髓炎及根尖周炎患牙的根管治疗期间急症发生率小,值得推广。Objective To compare the effects of two different instruments on pain during root canal treatment in patients with postoperative pulpitis and periapical periodontitis.Methods A total of 150 patients who required root canal therapy after dry pulp surgery from January 2014 to December 2016 in our hospital were enrolled. The patients were divided into two groups for 75 cases in each one. The control group patients were treated with the stainless steel hand K-files with the step-down technique. And the patients in the observation group were treated by S3 nickel-titanium rotary file with crown down technique. The patients were followed up after root canal preparation, and the pain was recorded and compared.Results After root canal preparation, Endodontic Interappointment Emergencies(EIES) occurred on 24 teeth in the observation group, the incidence rate was 32%;in the control group, 48 teeth were developed EIES, the incidence rate was 64%, and the observation group which had pain Grade I(17.3%) Grade II(10.6%) Grade III(4%) were better than that of the control group(Grade I. 29.3%, Grade II 24%, Grade III 10.7%), statistical analysis χ~2=15.818, P<0.05, the difference is statistically significant.Conclusion Compared to the stainless steel hand K-files, S3 nickel-titanium rotary file has less EIES during the root canal preparation on dry pulp surgery about postoperative pulpitis and apical periodontitis. S3 nickel-titanium rotary file should be extended.
