三维适形放疗与手术分别联合TACE治疗PHC对患者血清肿瘤标志物水平的影响

摘　　要：目的探讨三维适形放射治疗、外科手术分别联合肝动脉栓塞化疗(TACE)治疗原发性肝癌(PHC)对患者血清肿瘤标志物水平的影响。方法 2016年4月—2018年3月新乡医学院第一附属医院收治的98例PHC患者临床资料,根据使用治疗方法分为研究组(三维适形放射治疗联合TACE治疗,n=51)与对照组(外科手术联合TACE治疗,n=47)。观察治疗前与治疗后1个月两组患者血清肿瘤标志物[甲胎蛋白(AFP)、细胞角蛋白19可溶性片段(CYFRA21-1)、α-L-岩藻糖苷酶(AFU)],比较治疗后1年及治疗后2年两组患者生存率及复发率,并分析治疗后1个月内两组患者不良事件发生情况差异。结果治疗后2个月,两组患者血清AFP、CYFRA21-1及AFU水平均较治疗前显著降低,且研究组明显低于对照组(P<0.05);治疗后1年及治疗后2年,研究组患者生存率(96.08%,88.24%)均明显高于对照组(80.85%,68.09%),差异有统计学意义(P<0.05),研究组患者复发率均明显低于对照组(P<0.05);治疗后1个月内,两组患者各项化疗不良反应、急性放射损伤及术后并发症发生率比较均差异无统计学意义(P>0.05),但研究组患者不良事件总发生率明显低于对照组(P<0.05)。结论三维适形放射治疗联合TACE治疗可有效治疗PHC,提高患者生存率、减少复发,且安全性良好。Objective To explore the effect of three-dimensional conformal radiotherapy and surgery combined with hepatic artery embolization chemotherapy(TACE) for primary liver cancer(PHC) on serum tumor marker level.Methods The clinical data of 98 PHC patients admitted to our hospital from April 2016 to March 2018 were divided into study group(3 d conformal radiotherapy combined with TACE treatment, n=51) and control group(surgery combined with TACE treatment, n=47) according to the treatment methods used. Observed before and after treatment 1 month two groups of patients with serum tumor markers[armor fetoprotein(AFP), soluble cytokeratin 19 fragment(CYFRA21 1), the alpha-L-fucus glycosidase(AFU)], is 1 year after treatment, and 2 years survival rate and the recurrence rate in both groups after treatment, and analyze the 1 months after the treatment of adverse events in patients with two groups of differences.Results Two months after treatment, serum AFP, CYFRA21-1 and AFU levels in the two groups were significantly lower than those before treatment, and the levels in the study group were significantly lower than those of the control group(P<0.05). 1 year after treatment and 2 years after treatment, the survival rate of patients in the study group(96.08%, 88.24%) were significantly higher than those of the control group(80.85%, 68.09%), with statistically significant difference(P<0.05). The recurrence rate of patients in the study group was significantly lower than that of the control group(P<0.05). Within 1 month after treatment, there were no significant differences in adverse reactions of chemotherapy, acute radiation injury and postoperative complications between the two groups(P>0.05), but the total incidence of adverse events in the study group was significantly lower than that of the control group(P<0.05).Conclusion Three-dimensional conformal radiotherapy combined with TACE can effectively treat PHC, improve the survival rate of patients, reduce recurrence, and is safe.
