卡马西平联合丁苯酞软胶囊对脑卒中后癫痫患者认知功能的影响

摘　　要：目的分析脑卒中后癫痫联合应用卡马西平与恩必普治疗的效果及对其认知功能的影响。方法选取120例2018年1月—2020年1月驻马店市第二人民医院收治的脑卒中后癫痫患者,随机分为两组,对照组60例予以卡马西平治疗,研究组60例予卡马西平结合恩必普治疗,比较两组治疗后疗效、认知功能、脑源性神经营养因子(BDNF)、神经元特异性烯醇化酶(NSE)、生活质量及不良反应发生情况。结果研究组治疗有效率91.67%,高于对照组的73.33%,差异有统计学意义(P<0.05)。研究组治疗后认知功能评分(28.93±2.46)分,BDNF水平为(25.81±4.03)ng/mL,NSE水平为(5.68±1.65)ng/mL,分别与对照组的(24.82±2.15)分、(18.67±3.85)ng/mL,(8.42±1.98)ng/mL比较,差异有统计学意义(P<0.05)。研究组治疗后心理功能评分(87.85±4.62)分,躯体功能(88.75±4.39)分,社会功能(79.63±5.41)分,物质生活评分(85.64±4.98)分,与对照组各维度评分比较差异有统计学意义(P<0.05)。两组不良反应发生率分别为10.00%和6.67%,差异无统计学意义(P>0.05)。结论卡马西平与恩必普联合应用,能够有效提升临床对脑卒中后癫痫的治疗效果,加速患者认知功能和生活质量改善,且安全性高,值得推广。Objective To analyze the effect of combined treatment of carbamazepine and empiple on epilepsy after stroke and its influence on cognitive function.Methods Chose 120 cases from January 2018 to January 2020 epilepsy after stroke patients of our hospital, were randomly divided into two groups, control group 60 cases with carbamazepine treatment, group of 60 cases with carbamazepine treatment, combined with grace will compare the two groups after treatment efficacy, cognitive function, BDNF, NSE, life quality and adverse reactions occur.Results The effective rate in the study group was 91.67%, higher than 73.33% in the control group, the difference was statistically significant(P<0.05). The BDNF and NSE levels in the study group were(25.81±4.03) ng/mL and(5.68±1.65) ng/mL after treatment(28.93±2.46), which were significantly different from those in the control group(24.82±2.15),(18.67±3.85) ng/mL and 8.42±1.98 ng/mL(P<0.05). The scores of psychological function(87.85±4.62), physical function(88.75±4.39), social function(79.63±5.41), and material life(85.64±4.98) in the study group after treatment were significantly different from those in the control group(P<0.05). The incidence of adverse reactions in the two groups was 10.00% and 6.67%, respectively, with no statistically significant difference(P>0.05).Conclusion The combined application of carbamazepine and Empip can effectively improve the clinical treatment effect of epilepsy after stroke, accelerate the improvement of patients’ cognitive function and quality of life, with high safety, and is worthy of promotion.
