虹膜拉钩在晶状体半脱位超声乳化手术中的应用效果分析

摘　　要：目的探讨虹膜拉钩在半脱位晶状体超声乳化手术方面有效性和安全性。方法对晶状体半脱位白内障病人的临床资料进行汇总,选出48人纳入研究。以随机算法为工具均分成2组,对照组24人,开展超声乳化手术治疗;观察组24人,在手术中辅以虹膜拉钩。结果观察组手术情况优于对照组,23位病人成功植入人工晶体,且晶体居中率为82.61%,瞳孔居中率87.50%,仅有4位病人发生玻璃体脱出情况。与对照组的数据对比,P<0.05;手术前两组病人视力水平相差无异,手术后,观察组病人视力明显改善,20人视力升至0.3以上。与对照组对比,P<0.05。结论在半脱位晶状体超声乳化手术中辅以虹膜拉钩,术中合理的超声乳化参数,能够较好的完成悬韧带小于180°以内半脱位白内障病人的超声乳化手术。病人术中较少出现玻璃体脱出,人工晶体植入率较高,瞳孔居中良好。术后效果满意,具有推广价值。Objective To investigate the efficacy and safety of iris retractor in phacoemulsification of subluxated lens.Methods The clinical data of cataract patients with subluxation of lens were summarized,and 48 patients were selected in the study.With the random algorithm as the tool,they were divided into 2 groups,and the control group consisted of 24 patients.In the observation group,24 patients were treated with iris retractor.Results The operation in the observation group was better than that in the control group,23 patients were successfully implanted with iol,and the median rate of iol was 82.61%,the median rate of pupil was 87.50%,and only 4 patients had vitreous detachment.Compared with the control group,there were significant difference,P<0.05;There was no difference in visual acuity between the two groups before the operation.After the operation,the visual acuity of the observation group was significantly improved,with 20 patients’visual acuity rising to more than 0.3.Compared with the control group,there were significant difference,P<0.05.Conclusions In phacoemulsification surgery with iris retractor,the reasonable phacoemulsification parameters during the operation can better complete the phacoemulsification surgery for cataract patients with subluxation within 180°suspension ligament.Intraoperative vitreous detachment was rare,intraocular lens implantation rate was high,and the pupil was well centered.The postoperative results are satisfactory and have the promotion value.
