摘  要:目的:探讨精神分裂症患者脑源性神经营养因子(BDNF)与事件相关脑电位N400和临床症状之间的关系。方法:对58例精神分裂症患者(患者组)及60名健康对照(正常对照组)进行N400测查,采用酶联夹心免疫吸附法测定血清BDNF水平。使用阳性和阴性症状量表(PANSS)评定患者的临床症状。结果:患者组血清BDNF水平低于正常对照组,差异有统计学意义(P<0.05)。在Fz点,患者组N400中异音形似异义的匹配潜伏期延迟(P<0.05);N400中异音异形同义的匹配潜伏期和波幅以及非匹配波幅下降(P<0.05或P<0.01)。BDNF水平变化值与PANSS总分及各因子分变化值无相关性(P均>0.05)。患者组血清BDNF水平与Fz点N400波幅与潜伏期呈正相关(P<0.05或P<0.01)。结论:精神分裂症患者血清BDNF水平降低与其N400变异及认知功能损害有关。Objective:To explore the relationship between brain-derived neurotrophic factor(BDNF),events-related potentials(ERP)N400 and clinical symptoms in patients with schizophrenia.Method:58 patients with schizophrenia(patient group)and 60 normal controls(control group)were enrolled with record and analysis of N400.The serum BDNF level was tested by enzyme-linked immunesorbent assay(ELISA),and the clinical symptoms of patients were assessed by Positive and Negative Symptoms Scale(PANSS).Results:BDNF level in the patient group was significantly lower than that in the control group(P<0.05).At Fz point,N400 latency elicited by words with different phonic,similar shape and different meaning delayed in the patient group(P<0.05);matching N400 latency and amplitude and non-matching N400 amplitude elicited by words with different phonic and shape and same meaning decreased(P<0.05 or P<0.01).There was no correlation between the change value of the BDNF level and the change value of PANSS total score and factor scores(all P>0.05).The serum BDNF level was positively correlated with N400 amplitude and latency of Fz point(P<0.05 or P<0.01).Conclusion:Decreasing serum BDNF level is related with N400 and cognitive deficit in the patients with schizophrenia.
