后腹腔镜下根治性肾切除术治疗肾癌的效果及对患者肾功能的影响

摘　　要：目的探讨后腹腔镜下根治性肾切除术治疗肾癌的效果及对患者肾功能的影响。方法选择2019年1月至2019年11月行外科手术治疗的63例肾癌患者为研究对象,根据手术方式不同将其分为开放组(30例)与腹腔镜组(33例)。开放组行传统开放性肾癌根治术,腹腔镜组行后腹腔镜下根治性肾切除术。比较两组的治疗效果。结果术后,两组的CRP、IL-6水平均升高,BUN、Cr水平均降低,且腹腔镜组优于开放组(P<0.05)。术后,两组的CEA、CA50水平均降低,且腹腔镜组低于开放组(P<0.05)。腹腔镜组的术后并发症总发生率低于开放组(P<0.05)。腹腔镜组的住院时间短于开放组,术中出血量少于开放组(P<0.05)。结论后腹腔镜下根治性肾切除术能减轻对机体的应激反应,降低肿瘤因子水平,利于患者肾功能恢复,且安全性高。Objective To investigate the effect of retroperitoneal laparoscopic radical nephrectomy in the treatment of renal carcinoma and its influence on renal function of patients.Methods A total of 63 patients with renal carcinoma who underwent surgical treatment from January 2019 to November 2019 were selected as the research objects.According to different surgical methods,the patients were divided into open group(30 cases)and laparoscopic group(33 cases).The open group underwent traditional open radical nephrectomy,and the laparoscopic group underwent retroperitoneal laparoscopic radical nephrectomy.The therapeutic effects of the two groups were compared.Results After operation,the levels of CRP and IL-6 in the two groups increased,the levels of BUN and Cr decreased,and those in the laparoscopic group were better than the open group(P<0.05).After operation,the levels of CEA and CA50 in the two groups decreased,and those in the laparoscopic group were lower than the open group(P<0.05).The total incidence of postoperative complications in the laparoscopic group was lower than that in the open group(P<0.05).The hospital stay of the laparoscopic group was shorter than that in the open group,and the intraoperative blood loss volume was less than that in the open group(P<0.05).Conclusion Retroperitoneal laparoscopic radical nephrectomy can reduce the stress response to the body and the levels of tumor factors,which is beneficial to the recovery of the patients'renal function and has high safety.
