摘  要:目的比较非小细胞肺癌(NSCLC)和肺内炎性肿物18 F-FDG代谢分布的差异。方法选择20例病理诊断明确的NSCLC和20例肺炎性肿物进行18 F-FDG显像和分析。以最大SUV值(SUVmax)的不同百分比作为阈值自动勾画感兴趣区(ROI),获得34%SUVmax到90%SUVmax阈值内病变组织的体积及18 F-FDG分布曲线。通过PMOD和MATLAB软件重建病变组织的18 F-FDG PET三维表面重建图、二维色温图及等高线图来进行比较。结果 (1)20例NSCLC和20例肺炎性肿物均可摄取18 F-FDG,两者SUVmax值有统计学差异(t=3.482,P<0.05)。(2)NSCLC18F-FDG PET 3D表面重建影像多近似球形,而肺炎性肿物形态多不规则(χ2=12.907,P<0.001)。(3)瘤体内高代谢区单一集中还是多发分散与肿物的性质无关(χ2=0.404,P>0.05),但受到肿物体积大小的影响(t=6.805,P<0.01)。(4)等高线图像显示,如果将80%SUVmax以上阈值区设为热点,在SUVmax层面NSCLC的热点多接近瘤体的几何中心,而炎性肿物多偏离瘤体的几何中心(χ2=4.912,P<0.05)。NSCLC18F-FDG不同阈值的等高线形态更加相似,与周围正常组织界限清晰。而炎性肿物中18 F-FDG不同阈值的等高线图形差别明显,与正常肺组织界限不易区分(χ2=10.000,P<0.05)。结论 NSCLC与肺炎性肿物的18F-FDG代谢分布存在差异,18F-FDG代谢分布影像有利于肺内肿物性质的诊断。Objective Compare the difference in the metabolic distribution of 18 F-FDG between NSCLC and inflammatory lung lesions.Methods Select 20 cases of NSCLC and 20 cases of inflammatory lung lesions with clear pathological diagnosis for 18 F-FDG imaging and analysis.Using different percentages of SUVmax as threshold to automatically draw ROI,obtain the lesion volume and 18 F-FDG distribution curve within the threshold of 34% SUVmax to 90% SUVmax.Using PMOD and MATLAB software to reconstruct the 18 F-FDG PET 3 D surface reconstruction map,color temperature map and contour map of the diseased tissue,and compare them.Results(1) Both 20 cases of NSCLC and20 cases of inflammatory lung lesions can take 18 F-FDG.The SUVmax values of the two are statistically different(t=3.482,P<0.05).(2)The 18 F-FDG PET 3 D surface reconstruction images of NSCLC and inflammatory lung lesions are approximately spherical,and the inflammatory lung lesions are more irregular in shape(χ2=12.907,P<0.001).(3)Whether the high-metabolism area in the tumor is concentrated or scattered has nothing to do with the nature of the tumor(χ2=0.404,P>0.05),but is closely related to the volume of the tumor(t=6.805,P<0.01).(4) The contour image show that at the SUVmax level,the hot spots of NSCLC 18 F-FDG metabolism are mostly close to the geometric center of the tumor,while that of inflammatory lung lesions are often away from the geometric center of the tumor(χ2=4.912,P<0.05).The 18 F-FDG distribution level of NSCLC is more clear,and the contours of different thresholds are more similar,while the distribution level of 18 F-FDG in inflammatory lung lesions are relatively confusing,and the contours of different thresholds are significantly different(χ2=6.667,P<0.05).Conclusion There is a difference in the18 F-FDG metabolic distribution of NSCLC and inflammatory lung lesions,which can be used to diagnose the nature of lung tumors.
