老年胃病患者Hp感染与否及其血清CEA、AFP及CA50表达及临床意义

摘　　要：目的研究老年慢性萎缩性胃炎(CAG)患者幽门螺杆菌(Hp)感染血清癌胚抗原(CEA)、甲胎蛋白(AFP)及糖链抗原50(CA50)表达及临床意义。方法回顾性分析宜宾市第一人民医院2017年10月-2019年8月期间收治的接受胃镜和病理检查的100例老年患者的临床资料。根据病理变化结果将患者分为CAG组(n=42)、肠上皮化生组(n=38)、不典型增生组(n=12)以及癌变组(n=8),比较不同病理变化组患者血清中CEA、AFP及CA50的表达水平以及Hp感染情况,并比较不同病理变化组Hp(+)患者和Hp(-)患者血清CEA、AFP及CA50的表达水平。结果癌变组患者血清CEA、AFP、CA50表达水平明显高于不典型增生组、肠上皮化生组、CAG组(P<0.05),不典型增生组与肠上皮化生组血清中CEA、AFP、CA50表达水平无统计学差异(P>0.05),但均高于CAG组(P<0.05);不同病理变化组患者Hp感染的阳性率比较有明显差异,病理变化越严重,Hp阳性率表达率逐渐升高,差异有统计学意义(P<0.05);不同病理变化组Hp(+)患者血清CEA、AFP、CA50表达水平要高于Hp(-)患者(P<0.05)。结论 Hp感染可能在胃黏膜癌变中发挥重要作用,CEA、AFP及CA50等肿瘤标志物水平与CAG病理改变和Hp感染有关。Objective To study expression and clinical significance of serum carcinoembryonic antigen(CEA),alpha-fetoprotein(AFP)and carbohydrate antigen 50(CA50)in elderly patients with chronic atrophic gastritis(CAG)and Helicobacter pylori(Hp)infection.Methods The clinical data of 100 elderly patients who underwent gastroscopy and pathological examination in the hospital from October 2017 to August 2019 were retrospectively analyzed.According to results of pathological changes,they were divided into CAG group(n=42),intestinal metaplasia(IM)group(n=38),atypical hyperplasia group(n=12)and cancerization group(n=8).The expression levels of serum CEA,AFP and CA50,and Hp infection were compared among different groups.The expression levels of serum CEA,AFP and CA50 in Hp(+)patients and Hp(-)patients were compared among different groups.Results The expression levels of serum CEA,AFP and CA50 in cancerization group were significantly higher than those in atypical hyperplasia group,IM group and CAG group(P<0.05).There were no significant differences in expression levels of serum CEA,AFP and CA50 between atypical hyperplasia group and IM group(P>0.05),higher than those in CAG group(P<0.05).There was no significant difference in positive rate of Hp infection among different groups.The more serious the pathological changes,the higher the expression positive of Hp infection(P<0.05).The expression levels serum CEA,AFP and CA50 in Hp(+)patients were higher than those in Hp(-)patients(P<0.05).Conclusion Hp infection may play important roles in gastric mucosal cancerization.The levels of tumor markers such as CEA,AFP and CA50 are related with pathological changes of CAG and Hp infection.
