视网膜母细胞瘤D期化疗联合激光光凝的效果

摘　　要：目的观察静脉化疗、经眼动脉灌注化疗联合激光光凝治疗D期视网膜母细胞瘤所有患者的效果。方法回顾性病例研究。分析郑州大学附属儿童医院2015年12月至2018年12月单眼视网膜母细胞瘤D期13例的临床资料,采取静脉化疗、经眼动脉灌注化疗和激光光凝治疗。随访6~60个月。观察治疗效果、视力及并发症。结果随访期内,10例肿瘤缩小或完全钙化,眼球保留情况稳定者占76.92%(10/13),3例因病情未能有效控制而行眼球摘除术占23.08%(3/13)。视力情况:4例无光感,2例光感,4例视力为0.02~0.4。并发症:眼睑、结膜水肿9例,轻度骨髓抑制5例,均于1个月左右减轻。结论静脉化疗、经眼动脉灌注化疗联合激光光凝治疗D期视网膜母细胞瘤临床效果较好,可致肿瘤消退、钙化,控制肿瘤进展,部分可保存视力,且并发症较少。Objective To observe the efficacy of intravenous chemotherapy,transophthalmic arterial infusion chemotherapy combined with laser photocoagulation for the treatment of retinoblastoma with stage D.Methods This was a retrospective case study.The clinical data of 13 patients of monocular retinoblastoma with stage D from Dec.2015 to Dec.2018 in Children’s Hospital Affiliated to Zhengzhou University were collected and analyzed.Intravenous chemotherapy,transophthalmic arterial perfusion chemotherapy and laser photocoagulation were used for treatment.The patients were followed up for 6-60 months.The efficacy,visual acuity and complications were observed.Results During the follow-up period,10 cases with tumor shrinkage or complete calcification had stable eyeball retention,accounting for 76.92%(10/13).And 3 cases were performed enucleation due to the failure of effective control of the disease,accounting for 23.08%(3/13).4 cases had light perception,2 cases had no light perception,and 4 cases had visual acuity of 0.02-0.4.Eyelid and conjunctival edema occurred in 9 cases,and mild bone marrow suppression occurred in 5 cases,all of which were alleviated in about 1 month.Conclusion Intravenous chemotherapy and transophthalmic arterial chemotherapy combined with laser photocoagulation have good clinical efficacy for the treatment of retinoblastoma with stage D,which can cause tumor regression and calcification,control tumor progression,partially preserve eyesight,and have fewer complications.
