酒精依赖所致睡眠障碍与认知功能损害的相关性研究

摘　　要：目的研究酒精依赖患者睡眠障碍和认知功能损害程度的关联性,为临床治疗酒精依赖患者提供有益指导。方法经急性期治疗后(1周),仍有睡眠障碍及认知障碍的酒精依赖患者240例作为研究对象,收集患者的临床资料,给予患者匹兹堡睡眠量表(PSQI)、汉密尔顿抑郁量表(HAMA)、汉密尔顿焦虑量表(HAMD)测试,给予患者认知功能评分测试,给予患者多导睡眠图(PSG)测定。按照睡眠质量分组,分为伴有睡眠障碍组和伴有严重睡眠障碍组,并开展相关性分析。结果伴有严重睡眠障碍组在语言流畅性测验、空间广度测验、言语记忆测试、视觉记忆测验方面均显著低于伴有睡眠障碍组(P<0.05)。PSQI、HAMD、HAMA评分方面显著高于伴有睡眠障碍组(P<0.05),多因素线性回归分析的结果显示,患者语义流畅性测验与年龄相关(β=-1.9822,P<0.05),患者空间广度测验与HAMD的睡眠障碍因子有关(β=-5.2842,P<0.05),患者言语记忆测验结果与婚姻状况有关(β=-1.0224,P<0.05);患者视觉记忆测验结果与年龄相关联(β=2.4775,P<0.05)。结论对于酒精依赖所致睡眠障碍与认知功能损害有着明显的相关性,因此,在临床治疗中,也应关注患者的认知功能损害情况,为患者安排合理的治疗方案,保证患者的临床治疗效果。Objective To investigate the correlation between sleep disorders and cognitive impairment in alcohol-dependent patients,so as to provide useful guidance for clinical treatment.Methods A total of 240 alcohol-dependent patients with sleep disorders and cognitive impairment after 1-week treatment at acute phase were enrolled in the study.The patients underwent PSQI scale,HAMA scale,HAMD scale test,cognitive function score test,and polysomnography(PSG)measurement.And the patients were divided into sleep disorder group and evere sleep disorder group according to sleep quality,and the correlation analysis was performed.Results The scores of language fluency test,spatial breadth test,verbal memory test,and visual memory test in severe sleep disorder group were significantly lower than those in sleep disorder group(P<0.05).However the scores of PSQI,HAMD,HAMA in severe sleep disorder group were significantly higher than those in sleep disorder group(P<0.05).The results of multivariate linear regression analysis showed that the patients’semantic fluency test results and visual memory test results were correlated with patient’s age(P<0.05),and patients’spatial breadth test was related to HAMD sleep disorder factor(P<0.05),and patients’verbal memory test results were related to marital status of patients(P<0.05).Conclusion There exists a close correlation between sleep disorders caused by alcohol dependence and of the cognitive function impairment of patients.Therefore,in clinical practice,it is necessary to pay much attention to the cognitive impairment of patients,to arrange reasonable treatment programs for patients,and ensure the clinical curative effects.
