摘　　要：面对越发严峻的人口老年化趋势以及急剧增长的养老医疗需求,发展以社区为平台的医养结合养老模式俨然已经成为有效解决我国医疗与养老问题的关键所在。基于此,该文以社区居家养老服务为研究视角,对社区居家养老服务医养结合存在的实际问题进行剖析,借助于互联网+信息化技术打造医疗护理与社区居家养老服务相互衔接的融合机制,以此为全面提升社区居家养老服务效能,满足人们的多样化养老需求奠定良好的基础。Faced with the increasingly severe population aging trend and the rapilly increasing demand for elderly care,the development of a community-based integrated medical care and elderly care model has become the key to effectivelysolving my country's medical and elderly care problems.Based on this,this article takes community home care services as aresearch perspective to analyze the actual problems in the integration of community home care services with medical care,andcreate medical care and community home care services with the help of Internet+information technology.The cohesiveintegration mechanism will lay a good foundation for comprehensively improving the efficiency of community home careservices and satisfying people's diverse elderly care needs.
