摘　　要：通过对“医院+社区+家庭”闭环模式在实际综合应用过程中相关环节的建立、推进、协调,简介推进的具体模式与效果。通过采用归纳法,对相关工作进行汇总分析,总结出相关环节有效结果。从组织上、平台建设上、制度制订、流程规范、活动开展各方面均取得良好效果。“医院+社区+家庭”闭环模式是众多模式中一项,是否有效,不能死搬硬套,应该有创新的改良,可以取得良好效果。Through the establishment,promotion and coordination of relevant links in the actual comprehensive application of the"hospital+community+family"closed-loop model,to introduce the specific model and effect of the promotion.By adopting the induction method,the relevant work is summarized and analyzed,and the effective results of the relevant links are summarized.Good results have been achieved in terms of organization,platform construction,system formulation,process standardization,and activity development.The closed-loop model of"hospital+community+family"is one of the many models,whether it is effective.It should be innovative and improved,and good results can be achieved.
