阳和汤加减联合美沙拉嗪缓释片治疗节段性回肠炎临床研究  

Effect of Using Yanghe Decoction Combined with Mesalazine Sustained Release Tablets in the Treatment of Segmental Ileitis

摘　　要：目的:观察阳和汤加减联合美沙拉嗪缓释片治疗节段性回肠炎对患者单核细胞趋化蛋白-1(MCP-1)、生长素释放肽(Ghrelin)的影响,为临床治疗提供参考。方法:选取我院2018年2月~2020年2月收治的节段性回肠炎患者94例,根据治疗方法分为两组,每组47例。对照组给予美沙拉嗪缓释片治疗,观察组给予阳和汤加减联合美沙拉嗪缓释片治疗。记录两组治疗前后CD活动指数(CDAI)评分、炎症性肠病问卷(IBDQ)评分及MCP-1、Ghrelin、血沉(ESR)、肿瘤坏死因子-α(TNF-α)、血清总蛋白(TLB)、白蛋白(Alb)、血小板(PLT)水平变化,并统计两组临床治疗总有效率。结果:观察组总有效率(97.87%)高于对照组(85.11%),差异具有统计学意义(P<0.05)。治疗后,两组TLB、Alb水平及IBDQ评分较治疗前上升,MCP-1、Ghrelin、ESR、TNF-α、PLT水平及CDAI评分较治疗前下降,观察组TLB、Alb水平及IBDQ评分高于对照组,MCP-1、Ghrelin、ESR、TNF-α、PLT水平及CDAI评分低于对照组(P<0.05)。结论:阳和汤加减联合美沙拉嗪缓释片治疗节段性回肠炎可减轻炎症反应,改善机体营养,有助于患者黏膜愈合。Objective: To observe the effect of using Yanghe Decoction combined with Mesalazine Sustained-release Tblets on monocyte chemoattractant protein-1(MCP-1) and auxin-releasing peptide(Ghrelin) in the treatment of segmental ileitis, and to provide reference for clinical treatment. Methods: 94 patients with segmental ileitis treated in our hospital from February 2018 to February 2020 were selected and divided into 2 groups according to the treatment method, 47 cases in each group. The controlled group was treated with Mesalazine Sustained-release Tablets, while the observation group was treated with Modified Yanghe Deoction combined with Mesalazine Sustained-release Tablets. The changes of CD activity index(CDAI) score, inflammatory bowel disease questionnaire(IBDQ) score and MCP-1,Ghrelin, erythrocyte sedimentation rate(ESR),tumor necrosis factor-α(TNF-α),serum total protein(TLB),albumin(Alb),platelet(PLT) before and after treatment. Results: The total effective rate in the observation group(97.87%) was higher than that in the controlled group(85.11%),and the difference was statistically significant(P<0.05). The TLB,Alb level and IBDQ score of the two groups were higher than before treatment, the MCP-1,Ghrelin, ESR,TNF-α,PLT level and CDAI score were lower than before treatment, the TLB,Alb level and IBDQ score of the observation group were higher than that of the controlled group, and the MCP-1,Ghrelin, ESR,TNF-α,PLT level and CDAI score were lower than that of the controlled group(P<0.05). Conclusion: Using Yanghe Decoction combined with Mesalazine Sustained-release Tablets in the treatment of segmental ileitis can alleviate the inflammatory response, improve the body nutrition, and help patients with mucosal healing.
