摘　　要：目的:观察陷胸三金汤对经内镜逆行性胰胆管造影术(ERCP)术后胆管炎的疗效及对TNF-α、IL-6、IL-8水平影响。方法:选取来我院接受治疗的98例ERCP术后胆管炎患者,随机分为两组各49例。对照组患者给予常规治疗,实验组在对照组基础上给予陷胸三金汤,两组患者均持续治疗30d。对比两组患者的临床疗效、血清AST、ALT、DBIL、TBIL水平、中医症候积分、TNF-α、IL-6和IL-8水平变化。结果:治疗后,实验组总有效率较高(P<0.05);两组血清AST、ALT、DBIL、TBIL水平明显降低(P<0.05);并且实验组降低较多(P<0.05);两组黄疸、腹痛、嗳气、食欲不振等症候评分显著降低(P<0.05);并且实验组降低较多(P<0.05);两组血清TNF-α、IL-6和IL-8水平明显降低(P<0.05);并且实验组降低较多(P<0.05)。结论:采用陷胸三金汤丁对ERCP术后胆管炎具有较好的治疗效果,能够降低TNF-α、IL-6、IL-8水平,安全性较高,值得在临床上推广应用。objective: to investigate the effect of baishuansan decoction on cholangitis after ERCP and its effect on TNF-α,IL-6 and IL-8 levels.Methods: a total of 98 patients with cholangitis after ERCP in our hospital were randomly divided into two groups, 49 cases each.Patients in the control group were given conventional treatment, and patients in the experimental group were given buxinsanjin decoction on the basis of the control group.Patients in both groups continued treatment for 30 days.Clinical efficacy, serum AST,ALT,DBIL and TBIL levels, TCM symptom score, TNF-α,IL-6 and IL-8 levels were compared between the two groups.Results: after treatment, the total effective rate of the experimental group was higher(P<0.05).Serum levels of AST,ALT,DBIL and TBIL were significantly decreased in the two groups(P<0.05).In addition, there was a significant decrease in the experimental group(P<0.05).The scores of jaundice, abdominal pain, belching and loss of appetite in the two groups were significantly reduced(P<0.05).In addition, there was a significant decrease in the experimental group(P<0.05).Serum levels of TNF-α,IL-6 and IL-8 were significantly decreased in both groups(P<0.05).In addition, there was a significant decrease in the experimental group(P<0.05).Conclusion: the treatment of ERCP patients with cholangitis after ERCP is effective and can reduce the levels of TNF-α,il-6 and il-8.
