摘  要:目的探讨急性脑梗死患者早期血清可溶性肿瘤坏死因子样弱凋亡诱导剂(Soluble tumor necrosis factor like weak apoptosis inducer, sTWEAK)水平与脑白质病变(White matter lesions, WMLs)的相关性。方法收集2018年2月-2019年12月在本院住院治疗的急性脑梗死患者184例作为研究对象,根据Fazekas量表评分将患者分成4组,比较各组患者的一般资料,并分析不同程度WMLs与sTWEAK水平的相关性。结果各组性别、高血压病史、糖尿病史的比例、TG,HDL水平比较无明显差异(P>0.05),年龄、冠心病史的比例、TC,LDL水平比较有明显差异(P<0.05);各组间sTWEAK水平比较有明显差异(P<0.01),且WMLs病情越重,sTWEAK水平越高;Pearson相关性分析发现,WMLs病变程度评分与年龄、sTWEAK水平呈明显正相关(r=0.421、0.485,P<0.05),与TC,LDL水平均呈明显正相关(r=0.523、0.401,P<0.05)。结论急性脑梗死患者早期血清sTWEAK水平与WMLs密切相关,随WMLs程度加重而升高,有望成为预测WMLs程度的潜在生物学指标之一。Objective To investigate the relationship between the early serum soluble tumor necrosis factor like weak apoptosis inducer(sTWEAK) level and white matter lesions(WMLs) in patients with acute cerebral infarction(ACI). Methods 184 patients with acute cerebral infarction who were hospitalized in our hospital from February 2018 to December 2019 were randomly selected as the research objects. According to the Fazekas scale score, the patients were divided into four groups. The general information of each group was compared, and the correlation between WMLs and sTWEAK levels in different degrees was analyzed. Results There were no significant differences in gender, hypertension prevalence, diabetes prevalence and TG expression level among the groups(P>0.05), but there were significant differences in age, coronary heart disease prevalence, TC expression level, HDL expression level and LDL expression level(P<0.05). There were significant differences in sTWEAK levels among different WMLs groups(P<0.01). Pearson correlation analysis showed that the severity of WMLs was positively correlated with age and sTWEAK(r=0.421, 0.485, P<0.05), and positively correlated with TC and LDL levels(r=0.523, 0.401, P<0.05). Conclusion The early serum sTWEAK level was closely related to WMLs in patients with acute cerebral infarction, and increased with the severity of WMLs. It was expected to become one of the potential biological indicators to predict the degree of WMLs.
