腹膜假黏液瘤1例报道及文献复习

摘　　要：目前关于腹膜假性黏液瘤(PMP)的来源、病理形态和诊断均有了较为完善的认识,较多回顾性分析研究结果亦肯定了细胞减灭术(CRS)联合腹腔热化疗(HIPEC)治疗PMP的效果。由于PMP的罕见性,CRS联合HIPEC的治疗价值尚未在前瞻随机研究中得到验证。虽然有少量研究提出了腹腔镜辅助CRS的可行性,但样本量较少,大多仍以开放手术为主。术前可应用肿瘤标志物和中性粒细胞淋巴细胞比率(NLR)对预后进行评估,但其效果还有待进一步验证。对于PMP预后的影响因素,HIPEC较肿瘤标志物检测结果或病理分型也未达成共识。因而部分研究仍对HIPEC的应用效果持不确定或不支持的观点。但全球医疗机构中支持CRS联合HIPEC策略的工作者数量呈上升趋势。The origin,pathological morphology and diagnosis of pseudomyxoma of peritoneum(PMP)have been well understood,and the results of more retrospective analysis also confirm the effect of cell subtraction(CRS)combined with peritoneal thermotherapy(HIPEC)in the treatment of PMP.the therapeutic value of CRS combined HIPEC has not been validated in prospective randomized studies due to the rarity of the PMP.Although a small number of studies have proposed the feasibility of laparoscopic assisted CRS,but the sample size is small,most of them are still open surgery.cancer markers and neutrophil lymphocyte ratio(NLR)can be used to evaluate the prognosis before operation,but the effect needs further verification.There is no consensus on the influencing factors of PMP prognosis HIPEC compared with the results of tumor markers or pathological classification.As a result,some studies still have uncertain or unsupported views on the effect of HIPEC application.Yet there is an upward trend in global health-care providers supporting CRS joint HIPEC strategies.
