经口径路与颈外径路舌根囊肿切除术临床对比研究

摘　　要：目的探讨经口径路与颈外径路舌根囊肿切除术的效果。方法前瞻性纳入2015-09—2019-09间郑州大学第一附属医院耳鼻咽喉头颈外科行舌根囊肿切除术的患者。根据手术入路分为经口径路组和颈外径路组。比较2组患者的临床资料(性别、年龄、围术期相关指标)和随访1~3 a期间的复发率。结果共纳入43例患者,经口径路组25例,颈外径路组18例。2组患者的性别、年龄差异无统计学意义(P>0.05)。经口径路组患者的手术时间、术中出血量、气管切开率、住院时间等围术期指标均短(少)于颈外径路组,差异有统计学意义(P<0.05),但随访期间的复发率高于颈外径路组,差异有统计学意义(P<0.05)。结论与颈外径路组相比,经口径路舌根囊肿切除术具有手术时间短、出血量少、气管切开率低、患者恢复快,以及颈部无切口瘢痕等优势,但其复发率较颈外径路组高。应根据患者的病情及术者的经验等予以选择。Objective To investigate the effect of transoral and external cervical keratectomy.Methods The patients who were Retrospectively included in otorhinolaryngology and head and neck surgery of the first affiliated Hospital of Zhengzhou University frpm 2015-09 to 2019-09 were divided into transcaliber group and cervical outer diameter group according to the surgical approach.The clinical data(sex,age,perioperative related index)and recurrence rate during follow-up 1~3 years were compared between the 2 groups.Results A total of 43 patients were included,25 patients in the transcaliber route group and 18 patients in the external cervical route group.There was no significant difference in sex and age between the two groups(P>0.05).The perioperative indexes of the patients with transcalibre road group were shorter(less)than those of the cervical road group(P<0.05),but the recurrence rate during follow-up was higher than that of the cervical road group(P<0.05).Conclusion Compared with the external cervical route group,transcalibular root cyst resection has the advantages of short operation time,less bleeding,low tracheotomy rate,quick recovery and no incision scar in the neck,but its recurrence rate is higher than that of the external cervical route group.Should be based on the patient’s condition and the experience of the operator to choose.
