乳腺原发性鳞状细胞癌与三阴性乳腺癌临床特征、治疗及预后的比较

摘　　要：目的:探讨乳腺原发性鳞状细胞癌(PPSCC)的临床特征、治疗及预后。方法:回顾分析作者所在两家医院2008年1月至2017年12月收治的15例乳腺PPSCC女性病例的资料,与同期收治的30例三阴性乳腺癌(TNBC)女性病例进行比较,并对研究对象进行随访,中位随访时间60个月。结果:15例乳腺PPSCC患者均接受了手术治疗:9例行乳腺癌根治术,其中3例术前行新辅助化疗;6例行保乳手术,其中2例术前行新辅助化疗。乳腺PPSCC患者与TNBC患者相比,年龄较长[(71.32±11.72)岁vs(57.24±10.31)岁,P=0.003],肿瘤直径较大[(43.00±28.47)mm vs(37.10±25.56)mm,P=0.032],复发较早(3个月vs 18个月,P=0.014),3年无病生存率(DFS)及5年总生存率(OS)均较短(3年DFS:57.3%vs 67.5%,P=0.036;5年OS:45.1%vs 62.4%,P=0.040)。结论:乳腺PPSCC与TNBC相比预后更差。Objective:To investigate the clinical features,treatment and prognosis of pure primary squamous cell carcinoma(PPSCC)of the breast.Methods:A retrospective analysis was performed on 15 female cases of PPSCC of the breast admitted between January 2008 and December 2017 to the First People s Hospital of Anqing City and the Third Affiliated Hospital of Guangzhou Medical University.These cases were compared to 30 female cases of triple-negative breast cancer(TNBC)admitted during the same period.Subjects were followed up,and the median follow-up time was 60 months.Results:All 15 PPSCC patients underwent surgery:9 underwent radical mastectomy,3 of which underwent neoadjuvant chemotherapy before surgery;the other 6 underwent breast-conserving surgery,2 of which underwent preoperative Neoadjuvant chemotherapy.Compared with the TNBC cases,patients with PPSCC of the breast were older in age[(71.32±11.72)years old vs(57.24±10.31)years old,P=0.003],had larger tumor diameters[(43.00±28.47)mm vs(37.10±25.56)mm,P=0.032],earlier relapses(3 months vs 18 months,P=0.014),worse disease-free survival(DFS)(3-year DFS:57.3%vs 67.5%,P=0.036)and overall survival(OS)(5-year OS:45.1%vs 62.4%,P=0.040),the differences were statistically significant(P<0.05).Conclusion:PPSCC of the breast has a worse prognosis than triple negative breast cancer.
