TCD发泡试验对西宁地区无先兆性偏头痛与右向左分流的相关性研究

摘　　要：目的探究经颅多普勒(TCD)发泡试验对地处中高海拔的西宁地区无先兆性偏头痛患者右向左分流(RLS)发生率的检测作用。方法选取2018年11月至2020年7月西宁市第一人民医院收治的偏头痛患者273例(观察组),同期健康体检者200例(对照组),均进行TCD发泡试验,观察RLS的发生率。结果观察组RLS阳性107例(39.2%),其中Ⅰ级分流28例(10%),Ⅱ级分流25例(10%),Ⅲ级分流24例(9%),Ⅳ级分流30例(11%)。对照组RLS阳性65例(32%),其中Ⅰ级分流22例(11%);Ⅱ级分流14例(7%),Ⅲ级分流11例(5%),Ⅳ级分流18例(9%)。观察组和对照组Ⅰ级分流比较差异无统计学意义(P>0.05),余数据差异均有统计学意义(P<0.05)。结论偏头痛患者RLS阳性率高于正常对照组,中高海拔地区正常人及无先兆性偏头痛患者RLS发生率更高。Objective To research the incidence rate of right-to-left shunt(RLS)in patients of migraine without aura in Xining through Transcranial Doppler Bubble Test.Methods 273 cases with migraine chosen as the case group in Xining First People’s Hospital from November 2018 to July 2020,and 200 healthy physical examinees as the control group.All the subjects recceived Transcranial Doppler Bubble Test in order to observe the incidence rate of RLS.Results 107 cases in the case group had RLS positive rate(39.2%),including 28 cases at level Ⅰ with positive rate 10.3%,25 cases at level Ⅱ(10.2%),24 cases at level Ⅲ(8.8%)and 30 cases at level Ⅳ(10.9%).65 subjects in the control group had RLS positive rate 32.5%,including 22 cases at level Ⅰ with positive rate 11%,14 cases at level Ⅱ(7%),11 cases at level Ⅲ(5.5%)and 18 cases at level Ⅳ(9%).There was no statistic difference at level I(P>0.05)between the two groups,while there were statistic differences in other sets of data(P<0.05).Conclusions The positive rate of RLS in the case group is higher than that in the control group.The incidence rate of RLS of patients of migraine without aura and of healthy people living at middle-high altitudes is rather high.
