翼状胬肉切除联合自体球结膜瓣移植术的治疗效果观察

摘　　要：目的探讨翼状胬肉切除联合自体球结膜瓣移植术的治疗效果。方法回顾性分析258例翼状胬肉患者的临床资料,所有患者均行翼状胬肉切除联合自体球结膜瓣移植术,分析患者的手术指标、术前及术后3个月的相关临床指标、临床疗效、并发症发生率及复发率。结果258例患者的手术时间为(20.47±1.39)min,切口长度为(3.00±0.21)mm,术中出血量为(1.15±0.50)ml,角膜上皮愈合时间为(4.42±1.53)d。术后3个月,角膜散光度(CAD)为(0.69±0.25)°,低于术前的(2.37±0.63)°,裸眼视力(UCVA)为(0.67±0.13),泪膜破裂时间(BUT)为(11.41±1.96)s,高于术前的(0.46±0.10)、(8.82±1.28)s,差异有统计学意义(P<0.05)。258例患者中,显效164例、有效94例,无效0例,治疗总有效率为100.00%。258例患者中发生感染、眼球运动障碍、角巩膜溃疡、复视等并发症4例,并发症发生率为1.55%;复发2例,复发率为0.78%。结论翼状胬肉切除联合自体球结膜瓣移植术的治疗效果显著,创伤小,术后角膜散光度低,裸眼视力恢复好,并发症发生率低,复发率低,且对泪液稳定性影响小,具有积极的临床意义。Objective To discuss the therapeutic effect of pterygium resection combined with autologous bulbar conjunctival flap transplantation.Methods The clinical data of 258 patients with pterygium was retrospectively analyzed.All patients underwent pterygium resection combined with autologous bulbar conjunctival flap transplantation.The surgical indicators,related clinical indicators before and 3 months after surgery,clinical efficacy,incidence of complications and recurrence rate were analyzed.Results The operation time of 258 patients was(20.47±1.39)min,the incision length was(3.00±0.21)mm,the intraoperative blood loss was(1.15±0.50)ml,and the corneal epithelial healing time was(4.42±1.53)d.3 months after surgery,the corneal astigmatism degree(CAD)(0.69±0.25)°was lower than(2.37±0.63)°before surgery,and the uncorrected visual acuity(UCVA)(0.67±0.13)and break-up time(BUT)(11.41±1.96)s were all higher than(0.46±0.10)and(8.82±1.28)s before surgery,and the difference was statistically significant(P<0.05).Among 258 patients,164 cases were markedly effective,94 cases were effective,and 0 case were ineffective,with total effective rate of treatment as 100.00%.Among the 258 cases,4 cases had complications such as infection,ocular dyskinesia,corneal and scleral ulcer,diplopia,and the incidence of complications was 1.55%;2 cases recurred,the recurrence rate was 0.78%.Conclusion Pterygium resection combined with autologous bulbar conjunctival flap transplantation has a significant therapeutic effect,with less trauma,low postoperative corneal astigmatism,good recovery of naked vision,low complication rate,low recurrence rate,and little impact on tear stability,which contains positive clinical significance.
