结膜入路睑袋整复术联合眶隔脂肪转移技术在眶周年轻化中的应用

摘　　要：目的探讨结膜入路睑袋整复术联合眶隔脂肪带蒂转移改善下睑膨隆及眶周衰老形态的临床效果。方法自2019年10月至2020年8月,辽宁省人民医院整形外科对53例睑袋伴泪沟畸形且皮肤无明显松弛的患者行结膜入路睑袋整复术,修剪眶隔脂肪,带蒂移植于眶下缘及上颌前间隙隧道,填充泪沟并增加深层颊内侧脂肪室的容量,观察术后效果。结果所有患者获随访3~12个月,术后下睑膨隆、泪沟畸形及颊中沟凹陷均得到明显改善,获得了良好的眶周年轻化效果。无术区感染、眼睑外翻、眼睑退缩等并发症发生。结论本术式适用于下睑膨隆且下睑皮肤无明显松弛的患者,术后恢复较快、并发症较少,下睑膨隆、泪沟畸形及颊中沟均得到了明显改善,患者对术后效果较满意。Objective To investigate the clinical effect of transconjunctival lower eyelid blepharoplasty and orbital septum fat transplantation in improving the morphology of lower eyelid and periorbital senescence. Methods From October 2019 to August 2020,53 patients diagnosed with eye bags with tear trough deformity and without obvious skin sagging were treated with transconjunctival lower eyelid blepharoplasty. The orbital septum fat was trimmed, pedicled and transplanted into the anterior maxillary space tunnel. The lacrimal sulcus was filled and the deep medial cheek fat compartment was increased. Results All patients were followed up for 3 to 12 months after operation, the lower blepharoptosis, tear trough deformity and middle buccal furrow depression were significantly improved, and good orbital rejuvenation effect was obtained. No complications such as infection, eyelid ectropion, eyelid retraction occurred. Conclusion This operation is suitable for the patients with eye bags and without obvious skin sagging, the postoperative recovery is quick, the complications are few, the postoperative morphology of lower eyelid and the middle buccal sulcus are all significantly improved.
