经口内入路固定眶隔脂肪矫正睑袋伴眶下凹陷

摘　　要：目的介绍睑袋修复眶隔释放术中一种新的固定眶隔脂肪的方法。方法自2018年7月至2019年11月,郑州人民医院整形外科对28例睑袋伴明显眶下组织凹陷者,采用眶隔释放法,并经口内入路对眶隔脂肪进行固定,填充眶下组织凹陷处。结果本组28例患者获随访3~14个月。所有患者睑袋均消失,眶下凹陷均得到明显改善;无下睑退缩、外翻及睑球分离等并发症发生;无下睑凹陷、睑袋复发;2例出现皮肤瘀斑,2周左右自行消失;1例出现鼻唇沟处硬结,局部按摩理疗后硬结消失。结论经口内入路固定释放的眶隔脂肪,对伴有眶下组织凹陷的睑袋矫正具有良好的改善效果。Objective To introduce a newmethod for fixation of orbital septum fat in the orbital septum release surgery of lower eyelid pouch. Methods From July 2018 to November 2019, 28 patients with lower eyelid pouch and obvious infraorbital tissue depression underwent orbital septum fat release. The fixation of orbital septum fat was conducted through intraoral approach and the depression of infraorbital tissue was filled. Results All patients were followed up for 3 to 14 months. The lower eyelid pouches in 28 patients were disappeared and the infraorbital depressions were significantly improved. No complications such as lower eyelid retraction, lower eyelid ectropion and eyelid and eyeball separation occurred. No recurrence of lower eyelid depression or lower eyelid pouch was found. The skin ecchymosis occurred in 2 patients and disappeared in 2 weeks. The induration of nasolabial fold occurred in 1 patient and disappeared after local massage therapy. Conclusion The fixation of orbital septum fat through intraoral approach has a good improvement in the correction of lower eyelid pouch and infraorbital tissue depression.
