准分子激光原位角膜磨镶术应用Pentacam检测角膜形态变化的研究

摘　　要：目的探讨Pentacam检查角膜形态变化对准分子激光原位角膜磨镶术(LASIK)矫治近视疗效的评估价值。方法2019年6月至11月选取我院行手术治疗的62例(117眼)近视或近视散光患者作为研究对象。所有患者均行LASIK术治疗,且在术前、术后7 d和术后1、3、6个月使用Pentacam眼前节分析仪检测角膜表面变异指数(ISV)、垂直不对称指数(IVA)、高度偏心指数(IHD)、高度不对称指数(IHA)、中心圆锥角膜指数(CKI)、圆锥角膜指数(KI),分析各指标对LASIK术治疗效果的评估价值。结果LASIK术治疗后,患者裸眼视力、眼压、屈光度均明显改善,与术前比较差异有统计学意义(P<0.05)。术后各时间点ISV、IVA、IHD、IHA均高于术前,而术后各时间点KI均低于术前,差异有统计学意义(P<0.05)。术后各时间点CKI与术前比较差异无统计学意义(P>0.05)。受试者工作特征曲线(ROC)分析显示,Pentacam测量的各指标对LASIK术矫治近视效果的评估价值由高至低为ISV、IVA、IHA、IHD、KI,曲线下面积分别为0.913、0.894、0.881、0.828、0.809。结论LASIK术矫治近视的临床效果显著,使用Pentacam眼前节分析仪检查角膜形态变化可作为LASIK手术及预后的参考依据。Objective To explore the value of corneal morphological changes detected by Pentacam in the treat-ment of myopia by laser in situ keratomileusis(LASIK).Methods From June 2019 to November 2019,62 patients(117 eyes)with myopia or myopic astigmatism who underwent surgery in our hospital were selected as the research objects.All patients were treated with LASIK.Before operation,7 days after operation,1 month after operation,3 months after operation and 6 months after operation,the corneal surface variation index(ISV),vertical asymmetry index(IVA),index of height decentration(IHD),height asymmetry index(IHA),central keratoconus index(CKI),keratoconus index(KI)were measured by Pentacam.The evaluation value of each index on the therapeutic effect of LASIK was analyzed.Results After LASIK,the uncorrected visual acuity,intraocular pressure and diopter were significantly improved,and the difference was statistically significant(P<0.05).ISV,IVA,IHD and IHA at different time points after operation were higher than those before operation,while KI at different time points after operation was lower than those before operation(P<0.05).Receiver operating characteristic(ROC)curve analysis showed that the evaluation value of Pentacam for the therapeutic effect of LASIK was ISV,IVA,IHA,PE and KI,and the area under the curve was 0.913,0.894,0.881,0.828 and 0.809,respectively.Conclusion The clinical effect of LASIK on myopia is significant.The corneal morphological changes detected by Pentacam anterior segment analyzer can be used as a reference for LASIK surgery and prognosis.
