摘　　要：目的分析胃癌患者癌组织中人类白细胞抗原-Ⅰ(HLA-Ⅰ)表达水平与免疫活性细胞反应及预后之间的相关性。方法选取我院2014年12月-2016年12月收治的临床资料完整、手术切除的胃癌组织(胃癌组织组)和配对正常胃组织(正常胃组织组)为研究对象,每组80例。利用免疫组化技术检测2组HLA-Ⅰ、CD11c+树突状细胞、CD45RO+记忆性T细胞、CD8+细胞毒性T淋巴细胞阳性表达情况,并分析HLA-Ⅰ表达水平与树突状细胞、记忆性T细胞、细胞毒性T淋巴细胞表达及胃癌预后的相关性。结果胃癌组织组HLA-Ⅰ、CD11c+树突状细胞、CD45RO+记忆性T细胞、CD8+细胞毒性T淋巴细胞阳性表达率均低于正常胃组织组(P <0.05)。HLA-Ⅰ的表达与淋巴结转移、肿瘤分化程度、TNM分期、CD11c+树突状细胞、CD45RO+记忆性T细胞、CD8+细胞毒性T淋巴细胞表达有关(P <0.05)。淋巴结转移、Ⅲ/Ⅳ期、3种免疫活性细胞阴性表达、HAL-Ⅰ阴性表达是影响胃癌患者预后的独立危险因素(P <0.05)。结论胃癌组织中HLA-Ⅰ阴性表达与胃癌局部浸润的免疫活性细胞反应降低有关,并且具有成为预警淋巴结转移及预后生物标志物的潜在价值。Objective To analyze the correlation of the expression level of human leukocyte antigen-Ⅰ( HLA-Ⅰ)in cancer tissues with immunocompetent cells response and prognosis in patients with gastric cancer. Methods Eighty cases of gastric cancer tissues( gastric cancer tissue group) and 80 cases of matched normal gastric tissue( normal gastric tissue group) with complete clinical data in our hospital from December 2014 to December 2016 were selected. The positive expression of HLA-Ⅰ,CD11 c + dendritic cells,CD45 RO + memory T cells and CD8 + cytotoxic T lymphocytes were detected by immunohistochemistry. The correlation of HLA-Ⅰ expression levels with expression of dendritic cells,memory T cells,cytotoxic T lymphocytes and prognosis of gastric cancer were analyzed. Results The positive expression rates of HLA-Ⅰ,CD11 c + dendritic cells,CD45 RO + memory T cells and CD8 + cytotoxic T lymphocytes in gastric cancer tissue group were lower than those in normal gastric tissue group( P < 0. 05). The expression of HLA-Ⅰ was correlated with lymph node metastasis,tumor differentiation,TNM stage,and expression of CD11 c + dendritic cells,CD45 RO + memory T cells and CD8 + cytotoxic T lymphocytes( P < 0. 05). Lymph node metastasis,stage Ⅲ/Ⅳ,negative expression of three kinds of immunocompetent cells and negative expression of HLA-Ⅰ were independent risk factors for prognosis of gastric cancer patients( P < 0. 05). Conclusion The negative expression of HLA-Ⅰ in gastric cancer tissue is related to decrease of immunocompetent cells response with local gastric cancer infiltration,which is of potential value for predicting lymph node metastasis and prognosis biomarkers.
