摘　　要：目的探讨宫腔镜下钬激光治疗子宫黏膜下肌瘤的效果及对性激素、性生活质量的影响。方法选取我院妇科2018年1月—2019年5月收治的子宫黏膜下肌瘤88例,按照治疗方法的不同分为观察组51例和对照组37例,观察组给予宫腔镜下钬激光治疗,对照组给予宫腔镜电切术治疗。比较2组手术时间、术中出血量、术后疼痛程度、术后并发症及术后住院时间、月经恢复正常时间。比较2组术前、术后3个月血清促卵泡生长激素(FSH)、黄体生成素(LH)、雌二醇(E 2)水平,比较2组术前、术后6个月性生活质量评分。结果与对照组比较,观察组手术时间、术后住院时间及月经恢复正常时间显著缩短,术中出血量显著减少,术后视觉模拟疼痛评分显著降低(P<0.01)。观察组术后并发症总发生率降低(P<0.05)。对照组术后3个月FSH、LH水平高于手术前及观察组(P<0.01),观察组术前术后FSH、LH、E 2水平比较差异无统计学意义(P>0.05)。与术前比较,2组术后6个月各项性功能评分均显著升高,且观察组高于对照组(P<0.05,P<0.01)。结论宫腔镜下钬激光治疗子宫黏膜下肌瘤除具有手术时间短、术中出血量少、疼痛轻、术后恢复快、安全性高的优势外,对患者性激素水平及性生活质量的影响较小。Objective To investigate the effect of hysteroscopic holmium laser on submucous myoma of uterus and its influence on sex hormones and quality of sexual life.Methods A total of 88 cases of submucous myoma of uterus in our hospital from January 2018 to May 2019 were selected and divided into observation group(n=51)and control group(n=37)according to different treatment methods.The observation group was treated with hysteroscopic holmium laser,and the control group was treated with hysteroscopic resection.The duration of operation,intraoperative blood loss,postoperative pain,postoperative complications,postoperative hospital stay and menstrual recovery time were compared between the two groups.The serum levels of follicle stimulating hormone(FSH),luteinizing hormone(LH)and estradiol(E 2)were compared between the two groups before and at 3 months after operation,and the sexual life quality scores of the two groups before and at 6 months after operation were compared.Results Compared with the control group,the duration of operation,postoperative hospital stay and menstrual recovery time of the observation group were significantly shorter,the intraoperative blood loss was significantly reduced,and the postoperative visual analogue scale(VAS)pain score was significantly lower(P<0.01).In addition,the total incidence of postoperative complications in the observation group was significantly lower(P<0.05).Compared with those before operation and those in the observation group,the levels of FSH and LH in the control group were significantly increased at 3 months after operation(P<0.01).There was no significant difference in the levels of FSH,LH and E 2 in the observation group before and after operation(P>0.05).Compared with those before operation,the scores of sexual function in the two groups were significantly increased at 6 months after operation,and the scores in the observation group were significantly higher than those in the control group(P<0.05,P<0.01)Conclusion In the treatment of submucous myoma of uterus,hysteros
