摘  要:目的研究抑郁障碍患者注意偏向的特点及高频经颅磁刺激对其影响。方法选取2018年3月—2019年5月我院收治的抑郁障碍患者64例,按治疗方法不同分为观察组和对照组,每组32例。观察组给予药物联合高频经颅磁刺激治疗,对照组给予药物联合高频经颅磁刺激伪治疗,均治疗4周。评估2组临床疗效,并利用眼动仪记录2组对悲伤、平静、高兴情绪图片的注视时间。结果观察组疗效优于对照组(P<0.05)。2组治疗前后对悲伤图片的注视时间均长于平静和高兴情绪图片(P<0.01)。与治疗前相比,2组治疗后对悲伤、平静、高兴情绪图片的注视时间均缩短,且观察组对悲伤图片情绪的注视时间短于对照组(P<0.05,P<0.01)。2组治疗前后平静和高兴情绪图片的注视时间组内比较差异无统计学意义(P>0.05)。结论抑郁障碍患者存在负性注意偏向,对于中性、高兴情绪可能无明显的注意偏向。高频经颅磁刺激联合药物治疗可以更好地改善抑郁症状和负性注意偏向。Objective To study the characteristics of attentional bias in patients with depressive disorder and the effect of high-frequency transcranial magnetic stimulation on it.Methods A total of 64 patients with depressive disorder admitted to our hospital from March 2018 to May 2019 were selected and divided into observation group(n=32)and control group(n=32)according to different treatment methods.The observation group was given drugs combined with high-frequency transcranial magnetic stimulation,while the control group was given drugs combined with high-frequency transcranial magnetic stimulation pseudo treatment.After 4 weeks of treatment,the clinical efficacy of the two groups was evaluated,and the fixation time of sad,calm and happy pictures in the two groups was recorded by eye movement instrument.Results The curative effect of the observation group was better than that of the control group(P<0.05).The fixation time of sad pictures was longer than that of calm and happy pictures before and after treatment(P<0.01).Compared with that before treatment,the fixation time of sad,calm and happy pictures in the two groups were reduced after treatment,and the fixation time of sad pictures in the observation group was shorter than that in the control group(P<0.05,P<0.01).There was no significant difference in the fixation time of calm and happy pictures between the two groups before and after treatment(P>0.05).Conclusion There is negative attentional bias in patients with depressive disorder,and there may be no obvious attentional bias for neutral and happy mood.High-frequency transcranial magnetic stimulation combined with drug therapy can better improve depressive symptoms and negative attentional bias.
