摘 要:目的探讨出血性脑卒中患者血浆和脑脊液中激素及内源性血管活性物质变化情况.方法将121例脑出血患者分为高血压性脑出血组(A组,78例)和自发性蛛网膜下腔出血组(B组,43例),50例无神经系统疾病的患者为对照组.检测各组患者血浆及脑脊液中促性腺激素释放激素、β-内啡肽、一氧化氮(NO)、降钙素基因相关肽(CGRP)水平.结果入院时A组和B组患者血浆和脑脊液中促性腺激素释放激素、β-内啡肽水平明显高于对照组,B组明显高于A组,差异均具有统计学意义(P<0.01).入院后各时间点A组、B组患者促性腺激素释放激素、β-内啡肽水平均明显高于对照组,B组明显高于A组,差异均具有统计学意义(P<0.01).入院时A组和B组患者血浆和脑脊液中NO、CGRP水平明显低于对照组,B组明显低于A组,差异均具有统计学意义(P<0.01).入院后各时间点A组、B组患者NO、CGRP水平均明显低于对照组,B组明显低于A组,差异均具有统计学意义(P<0.05).结论性腺激素释放激素、β-内啡肽、NO、CGRP均参与高血压性脑出血和自发性蛛网膜下腔出血患者的下丘脑应激反应,其中自发性蛛网膜下腔出血患者变化更明显.Objective To analyze the changes of hormones and endogenous vasoactive substances in plasma and CSF of patients with hemorrhagic stroke.Method 78 patients with hypertensive intracerebral hemorrhage were selected as the hypertensive intracerebral hemorrhage group,43 patients with spontaneous subarachnoid hemorrhage as the subarachnoid hemorrhage group,and 50 patients without nervous system disease after lumbar puncture as the control group.The levels of gonadotropin releasing hormone,β-endorphin,carbon monoxide(NO)and calcitonin gene-related peptide(CGRP)in plasma and CSF were measured.Results The levels of gonadotropin releasing hormone andβ-endorphin in plasma and cerebrospinal fluid in patients with hypertensive intracerebral hemorrhage and subarachnoid hemorrhage were significantly higher than those in the control group,and those in the subarachnoid hemorrhage group were significantly higher than those in the hypertensive cerebral hemorrhage group(P<0.01).The levels of gonadotropin releasing hormone andβ-endorphin in hypertensive intracerebral hemorrhage group and subarachnoid hemorrhage group were significantly higher than those in control group at each time point after admission,and the levels of gonadotropin releasing hormone andβ-endorphin in subarachnoid hemorrhage group were significantly higher than those in hypertensive cerebral hemorrhage group(P<0.01).The levels of NO and CGRP in plasma and cerebrospinal fluid of patients with hypertensive intracerebral hemorrhage and subarachnoid hemorrhage were significantly lower than those of the control group at admission,while those in the subarachnoid hemorrhage group were significantly lower than those in the hypertensive cerebral hemorrhage group(P<0.01).At each time point after admission,the levels of NO and CGRP in the hypertensive intracerebral hemorrhage group and subarachnoid hemorrhage group were significantly lower than those in the control group,and the patients in the subarachnoid hemorrhage group were significantly lower than those in the hyper
