半导体激光联合盐酸米诺环素辅助治疗瘘管型根尖周炎的效果观察

摘　　要：目的:观察半导体激光根管内照射后联合盐酸米诺环素根管内封药辅助治疗瘘管型根尖周炎的临床效果。方法:210例瘘管型根尖周炎患者随机分为研究组、对照1组及对照2组,每组各70例。在常规根管预备的基础上,研究组采用半导体激光根管内照射后联合盐酸米诺环素根管内封药消毒;对照1组采用盐酸米诺环素进行根管内封药消毒;对照2组则采用VITAPEX糊剂进行封药消毒,其余根管治疗步骤相同。经1年随访,观察临床效果。结果:治疗1月后,研究组的有效率均高于对照1组(χ^(2)=9.508,P=0.009)和对照2组(χ^(2)=20.464,P<0.001);治疗1年后,研究组的有效率高于对照2组(χ^(2)=13.350,P=0.001),差异具有统计学意义。结论:半导体激光根管内照射后联合盐酸米诺环素根管内封药辅助治疗瘘管型根尖周炎具有窦道愈合快、疗程短、远期效果好等优点,值得临床推广应用。[Objective]To observe the clinical effect of semiconductor laser combined with minocycline hydrochloride in the treatment of fistula periapical periodontitis.[Method]210 cases of fistula periapical periodontitis were randomly divided into study group,control group 1 and control group 2 with 70 cases in each group.On the basis of routine root canal preparation,the study group was sterilized by semiconductor laser irradiation combined with minocycline hydrochloride in the root canal.The control group 1 was sterilized by minocycline hydrochloride in the root canal and control group 2 was disinfected by Vitapex paste in the root canal.The other root canal treatment steps were the same to the study group.The clinical effects of the three groups were observed and compared after one year follow-up.[Result]After one month of treatment,the effective rate of the study group was higher than that of the control group 1(χ^(2)=9.508,P=0.009)and the control group 2(χ^(2)=20.464,P<0.001).After 1 year of treatment,the effective rate of the study group was higher than that of the control group2(χ^(2)=13.350,P=0.001),and the difference was statistically significant.[Conclusion]The treatment of fistula periapical periodontitis with semiconductor laser irradiation combined with minocycline hydrochloride has the advantages of rapid fistula healing,short course of treatment and good long-term effect,which is worthy of clinical application.
